Job purpose:

Acquaint individuals with satisfactory work habits, rules, and job skills as required by employer through explanation, demonstration, modeling, and practice.

Duties and Responsibilities:

  • Maintain comprehensive and accurate individual electronic case records according to Agency standards, adhering to all state funding compliance requirements.
  • Assist individuals with career planning, which may include, but is not limited to interview preparation, exploring job openings, and completing steps to fulfill employer required documentation/training.
  • Accurately complete individual attendance and billing sheets, ensuring there are service documentation notes to support all services being billed.
  • Attend and participate in in-service, agency and funding meetings as required.
  • Provide education, support, and training to employers regarding supervising people with disabilities.
  • Maintain open and professional communication with outside agencies as relevant to individual needs.
  • Exercise supervision of individual(s) at community-based worksites.
  • Serve as Case Manager to assigned caseload and other individuals as necessary. This may include but is not limited to providing physical assessment; crisis intervention; individual counseling; independent living skills, leisure activities; skill training; job-seeking skill teaching; site development and modeling a variety of occupations and social skills.
  • Support individuals to increase independence on the job and in community based on planning and support team service plan.
  • Other related duties and responsibilities as assigned.

Requirements

Qualifications:

  • High School Diploma.
  • Three years of experience working with persons with disabilities preferred.
  • Depending on assignment or case management responsibilities, may be required to communicate in another language and understand another culture.
  • Valid Driver’s License and must maintain a satisfactory driving record.
  • You are required to be in possession of a car on-site during your entire shift to transport yourself and/or individuals served to community sites in all weather conditions and throughout all neighborhoods.
  • Depending upon the assignment must be able to work in a variety of settings, weather conditions and locations.
  • Must maintain PMT and CPR certification and all other required trainings and certifications.
  • Knowledge and skills in Microsoft Office and the internet.

Physical Requirements:

  • May require the ability to transfer individuals requiring personal hygiene care.
  • May require the physical endurance to use physical crisis intervention techniques which may include retraining individuals.
  • May provide physical interventions which may include lifting, mobility, first aid, and safety evacuations.
  • Move and lift weights up to 50 lbs. on an occasional basis.
