About the Role

Job Summary

UW Health in northern Illinois is seeking a flexible and experienced Advanced Practice Provider (APRN or PA-C) to join our Family Medicine Float Pool in Rockford, Illinois. This is an excellent opportunity for a clinician who enjoys variety, adaptability, and practicing in diverse care settings.

Your clinical support includes the following:

  • Variety in Practice – Rotate across multiple UW Health northern Illinois clinic locations, caring for a broad range of patients and communities

  • Diverse Clinical Experience – Provide care in both outpatient Family Medicine clinics and Immediate Care settings, offering a dynamic mix of practice

  • Adaptability Valued – Be part of a team that thrives on flexibility and the ability to step into different practice sites as needed

  • Work-Life Balance – Flexible weekday schedule, Monday through Friday with no weekend requirement

  • Strong Support System – Comprehensive onboarding program, collegial provider network, and mentorship for those newer to Family Medicine

Qualifications:

  • Master’s degree from accredited PA program or completion of a master’s level advanced practice nurse program.
  • Active APRN or PA license in State of IL and active DEA license or ability to apply for such license prior to or upon hire.
  • Two years of experience in Family Medicine
