The primary function of the LPN / LVN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN / LVN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.
Requirements
- Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.
- Coordinate care with the interdisciplinary group, patient/family, and referring agency.
- Assist in the development and implementation of patient care plans.
- Monitor patient health and report any changes to the supervising RN.
- Administer medications and treatments as prescribed.
- Educate patients and their families on health care needs, conditions, and options.
- Maintain accurate and timely documentation of patient care and progress.
Job Qualifications:
- Graduate of an accredited School of Vocational/Practical Nursing.
- Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license.
- Two years’ experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.
- HCHB experience highly preferred
- Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.
- Reliable transportation and valid, current auto liability insurance.
- Work in patients' homes in various conditions.
- Ability to work a flexible schedule and travel locally.
- Ability to lift up to 50 lbs and move patients.
- Requires working under some stressful conditions to meet deadlines and patient needs.
- Ability to make quick decisions and acquire necessary resources.
- Must meet patient/family individualized psychosocial needs.
