Serving others is what we were meant to do.</p> <p>Join Reliant at Home’s winning culture - named Fortune TOP TEN in the USA Best Workplaces for Aging Services and certified Great Place to Work 2020-2021, 2021-2022, and 2022-2023!<strong><br><br>Responsibilities:</strong></p> <ul> <li>Assists in identifying the patient’s physical and environmental needs as evidenced by documentation.</li> <li>Provides effective communication to the patient, family, team members and other health care professionals as evidenced by clinical notes, case conferences and communication notes.</li> <li>Calls doctor’s offices to obtain and review orders.</li> <li>Perform covid tests in-office as needed.</li> <li>Works on pre-claim reviews.</li> <li>Coordinate care between nurses and patients.</li> <li>Assists with field visits as needed.</li> <li>Assists with orders management.</li> <li>Assists with wellness calls.</li> <li>Order supplies.</li> <li>Work under supervision of DON.</li> <li>Other duties as assigned.</li> </ul> <p><strong>Requirements</strong></p> <ul> <li>Strong computer and data entry skills.</li> <li>Prior QA experience a plus.</li> <li>Able to multitask, strong sense of urgency, attention to detail.</li> <li>Comprehensive knowledge of the function and organization of the home health agency.</li> <li>Knowledge of current rules and regulations in Home Care.</li> <li>Ability to facilitate quality control issues.</li> </ul> <p><strong>Job Knowledge:</strong></p> <ul> <li>Comprehensive knowledge of the function and organization of the home health agency.</li> <li>Knowledge of&nbsp;current rules and regulations in Home Care.
