The Individual Support Worker provides supervision and counsel to individuals to foster their growth and development. They act as case managers, maintain records, and ensure that individuals' needs are met according to their Plan of Service.

What You’ll Do

· Serve as a case manager for your assigned individuals, helping to guide their goals, supports, and daily needs

· Coordinate services, appointments, and plans of care; ensure follow-through on all recommendations

· Build strong relationships with families, guardians, and interdisciplinary team members

· Support individuals in community activities, recreational programs, and daily living routines

· Maintain clear and accurate documentation, including program data, notes, billing, and medical information

Requirements

What We’re Looking For

· High school diploma or equivalent

· Valid driver’s license and reliable vehicle, ability to possess a State of CT Public Service License

· Advanced computer skills (Microsoft Office, data entry, billing

· Attention to detail

· Compassion, patience, flexibility, and sound judgment

· Ability to lift up to 50 pounds
