The Adult and Youth Case Manager plays a crucial role in working with individuals across a wide age range, from ages 5 to 21 and 21 and up, as well as their caregivers. The primary objective is to identify, assess, and address the unique needs and goals of clients to improve their overall well-being. This multifaceted role encompasses various areas of focus, including but not limited to mental health, physical health, economic needs, education, and positive social development.
In this position, the Case Manager provides ongoing support and guidance to clients on a weekly basis, both within the community and in their homes. Transportation may be required to facilitate essential connections to community resources and services. Collaborating with other service providers and professionals is essential to ensure a comprehensive and integrated approach to client care.

Job Type: Full-time
Salary: Starting from $44,778.24 - $52,241.28 (depending on applicant's experience and education - could be more if master's degree holder)

Requirements
Requirements for the role:
- Minimum of 1 year youth (working directly with youth under 18) and adult work experience required. Volunteer/internship experience may be counted.
- Willingness to handle male youth/adult clients
- Minimum of 4-year degree (related human service degree preferred but not required)
- Valid Driver's License for past 5 years
Ability to commute/relocate:
- Westminster (Carroll County), MD: Reliably commute or planning to relocate before starting work (Required)
Education:
- Bachelor's (Required)
Schedule:
- Day Shift (Required) - but will require some early evenings (at least 2-3 times a week)
Work Location:
- In person
- First 2-4 weeks of training will be in our Hagerstown office (Required). Stipend would be provided - to be further discussed upon hire.
