About the Role

The Monash Health Nephrology Nurse Practitioner role is integral to the multidisciplinary team that services the health care needs of people with chronic kidney disease stage 3B to 5 in the south-east corridor of metropolitan Melbourne and regional Victoria. The position may be based at any of the key Monash Health Nephrology sites depending on service needs and will usually have exposure to multiple sites with core and negotiated elective sub-specialty areas of responsibility:

Core:

  • Nephrology Nurse Practitioner Clinics
  • Renal Outpatient Clinics
  • Satellite Dialysis Units
  • Quality Improvement

Elective:

  • Supportive Care
  • Kidney Failure and Diabetes
  • Home Dialysis
  • Kidney Transplantation

The scope of this role will include all core requirements and at least one other elective by negotiation and with Director of Nephrology approval.

 

About Monash Health Nephrology

Monash Health is the quaternary provider of Nephrology Services in the south east corridor of metropolitan Melbourne and regional Victoria. Services include chronic kidney disease prevention and management, kidney and kidney-pancreas transplantation, a full suite of extracorporeal and peritoneal dialysis kidney replacement therapies, supportive care and comprehensive Nephrology research, education and training programs.

Monash Health provides a dialysis service under a hub and spoke (satellite) model with the hub service located at Monash Health Clayton Campus. Dialysis services are designed and located to support a range of patient dialysis needs including inpatient, home and satellite dialysis.  Under the DHHS Vic Renal Capability Framework 2019, Monash Health is a level 6 Hub provider with governance and support system responsibilities for its regional and rural dialysis partners.

You have AHPRA Nurse Practitioner endorsement and are ready to take this next step in your career. You lead by example, thrive working in a dynamic, fast paced environment and are truly passionate about clinical excellence and promoting and delivering evidence based and multidisciplinary care to all Nephrology patients. You are ready to join our leadership team and to contribute to the ongoing development of patient centred Nephrology Services at Monash Health.
