Are you looking for a meaningful opportunity in your own home? Become a Host Home / Shared Living provider with Vibrant SCL.

As an Independent Contractor, your primary responsibility is to maintain a safe home environment for the individual residing with you. Must reside in the state of Iowa due to state funding and own or rent your home.

You will provide guidance, support, and care for the individual living in your home. You will be working with the individual on goals which could include things like, shopping, budgeting coping skills, cooking meals, personal hygiene, and assisting them in ways to integrate into their community. You will be an advocate/mentor helping them to live their best life possible.

This partnership allows you to establish a meaningful relationship with the person you are hosting as you invite them to become part of your family.

Responsibilities include:

  • Provide support with ADL’s and goals designed for the individual residing in your home up to 24 hours a day.
  • Medication administration.
  • Medical appointments.
  • Support with overall health and well-being.
  • Daily documentation.
  • Provide fun opportunities of community inclusion, integration, and socialization.
  • Compliance with all state/agency policy and procedures.
  • Communicate effectively with team members as needed.
  • Support physical needs of individual served.
  • Provide transportation.
  • Assistance with bathing, toileting, feeding, behavioral support, etc.
  • Provide support for medical protocols, such as seizure, fall, feeding tube, etc. (If needed)
  • Care for someone with complex physical/medical needs (i.e., hearing impaired, vision impaired, non-ambulatory, etc.)
  • Potential support for behavioral issues.


Requirements

Requirements:

  • Must have reliable transportation, valid driver's license, car insurance, and current registration.
  • Must have active home/renters’ insurance.
  • Must be at least 18 years of age.
  • Must reside in the state of Iowa.
  • Must have a private bedroom available for the person you will be sharing your home with.
  • Must complete all state mandated training.
  • Environmentally safe and clean residence.
  • Ability to pass State/Federal and APS/CPS background check.
  • Experience preferred but not required.
  • Reliable phone or computer with internet access.
  • Must comply with the documentation policy before getting paid for the services that have been provided to the individual in your home.
  • Must be willing to have staff come unannounced, and announced for home visits to check to see how you and individual are doing.
  • Must be willing to sign an agreement as a contractor and follow agreed-upon statutes laid out in the agreement.
