General Description
The Centralized Intake Specialist serves as a vital first point of contact for individuals seeking support through our organization. This role is responsible for conducting comprehensive intake screenings, assessing client needs, and facilitating timely referrals to appropriate internal programs and external community resources. The ideal candidate will demonstrate empathy, discretion, and a strong understanding of social service systems.
Essential Duties and Responsibilities
- Conduct structured intake interviews with clients to gather relevant personal, social, and health-related information
o Provide accurate and timely referrals to internal programs and external community partners
o Initiates all necessary paperwork in compliance with agency and regulatory standards.
o Completes an identity verification process in accordance with agency and department protocols.
o Refers clients to appropriate community services or case management, advocating on client’s behalf when appropriate.
o Maintains current working knowledge regarding available client resources.
o Collaborate with program staff and community agencies to ensure continuity of care and service coordination
o Assists team members with requests for additional information from referral sources.
o Monitor follow-up outcomes and assist with service navigation as needed
- Maintains data required
o Enter and update demographic and insurance data for new clients enrolling or participating in agency programs into the appropriate system(s).
o Maintain detailed and confidential documentation in accordance with organizational policies and compliance standards
o Compiles necessary information to generate reports regarding client and program status and quality assurance as needed.
- Participates in relevant Agency meetings and/or trainings
- Actively pursues development of professional competencies related to job performance through reading, supervision, in-service training and attendance at conferences and workshops.
- Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse
- Other duties as assigned

Requirements
Qualifications
Education: Bachelor’s degree in social work or related field preferred. Equivalent Combination of education and experience will be considered. Bilingual (Spanish) preferred.
Experience: Minimum of two years of experience in intake, case management or social services preferrable. Must have customer/client service experience.
