Job Summary
The Community Engagement Coordinator - CHW for School-Based Health will support the planning, coordination, and delivery of health programs within schools and youth-serving organizations. This role strengthens the connection between Care Alliance Health Center and students, families, educators, and community partners by coordinating on-site health activities, providing health education, supporting clinical engagement, and ensuring students have direct access to Care Alliance services. The coordinator will also assist with outreach logistics, track outcomes, maintain strong partner relationships, and ensure programs reflect the diverse cultural needs of the communities we serve.

Key Responsibilities:

Program Development & School Partnership Support

  • Collaborate with the Outreach & Engagement  Manager, school administrators, and youth-serving partners to identify      student and family needs.
  • Collaborate with the Communications team to develop program materials, health education content, and engagement resources tailored for school environments.
  • Assist in planning annual school-based initiatives such as health fairs, vaccination events, parent resource nights, and mental-health support activities.

Program Coordination & On-Site Support

  • Schedule, coordinate, and implement school-based health programs, screenings, and educational activities.
  • Manage logistics for on-site events, including space setup, equipment needs, workflow design, and materials preparation.
  • Support clinical outreach teams during school events by assisting with health education and basic screenings as permitted by training and scope.
  • Ensure smooth coordination between Care Alliance providers, school staff, and community partners during events.

Community Health Worker (CHW) & Patient Services Representative (PSR) Responsibilities

• Provide culturally informed health education to students and families on topics such as preventive care, chronic disease, mental health, nutrition, and social determinants of health.
• Conduct outreach to help students and families establish a medical home with Care Alliance, including appointment scheduling, MyChart support, and referrals.
• Perform CHW assessments to identify social needs and connect families to internal and external resources (e.g., food access, housing support, behavioral health services, insurance navigation, and safety concerns).
• Support warm handoffs between school partners, Care Alliance clinicians, behavioral health teams, and care coordination staff.
• Serve in a PSR capacity during school-based outreach and on-site events to support patient access, registration, and intake workflows.
• Complete patient registration and intake tasks, including demographic verification, insurance information, consent documentation, and required forms in school-based settings.
• Support new patient enrollment and re-engagement of existing patients identified through outreach activities
• Schedule medical, dental, and behavioral health appointments for students and family members during or following school-based programming.
• Accurately document outreach interactions, registration activities, social needs, referrals, and follow-up actions in Care Alliance data systems, including the electronic health record (EHR), to support reporting and audit requirements.
• Verify insurance eligibility when applicable and provide information related to sliding fee scale and financial counseling resources in alignment with Care Alliance policies.
• Assist in care coordination activities such as reminder calls, follow-up outreach, barrier identification, and continuity-of-care support.
• Maintain confidentiality and adhere to HIPAA and FERPA standards at all times in school-based and youth-serving environments.
• Support clean workflows, data accuracy, and documentation practices that strengthen compliance, operational efficiency, and audit readiness.

Student, Family & Partner Engagement

  • Build and maintain strong relationships with school leadership, teachers, counselors, parent coordinators, and families.
  • Serve as a point of contact for school partners, ensuring consistent communication, responsiveness, and alignment with school needs.
  • Promote Care Alliance services through outreach, classroom engagement, parent communication, and resource distribution.
  • Represent Care Alliance at school meetings, youth events, and community collaborations.

Communications & Outreach

  • Assist in developing youth-friendly and parent-focused outreach materials that promote Care Alliance services.
  • Communicate program updates, schedules, success stories, and needs to internal teams and external partners.
  • Support consistent branding, messaging, and engagement strategies across assigned schools.

Evaluation, Reporting & Continuous Improvement

  • Collect attendance data, screening counts, referral numbers, and engagement metrics for all school-based programming.
  • Maintain organized tracking systems that support grants, reporting requirements, and internal evaluations.
  • Use data to identify gaps, trends, and opportunities for strengthening school-based health programming.
  • Participate in program debriefs and contribute to the development of improved workflows and student-centered strategies.

Qualifications and Skills:

  • Bachelor’s degree in public health, Social Work, Community Development, or a related field.
  • Minimum of 2–3 years of experience coordinating community programs, outreach initiatives, or public health projects.
  • CHW certification (or willingness to obtain within six months).
  • Strong verbal and written communication skills.
  • Experience coordinating events and activities.
  • Budgeting and financial management skills.
  • Ability to prioritize and manage multiple tasks efficiently.
  • Skills in community needs assessment, program development, and program evaluation.
  • Relationship-building, leadership, and team supervision abilities.
  • Experience working with culturally informed populations, including communities with low-income and individuals      experiencing homelessness.
  • Flexibility to work evenings, weekends, and community-based events as needed.
