Case Manager – Floater, Supportive Housing

Reports to: HSS Senior Program Manager – Supportive Housing

FLSA Status: Non-Exempt, Full-Time

Department: Health & Social Services

Grade: P1–P3

Location: Various Serving Seniors properties throughout San Diego (East Village, Clairemont, Chula Vista, etc.)


Job Purpose

The Case Manager – Floater, Supportive Housing, works independently and collaboratively with a multidisciplinary team to ensure high-quality service delivery to older adults aged 55 and over. This role provides flexible, site-based support across multiple locations, offering individualized case management, resource navigation, and advocacy to promote healthful, stable living for residents in supportive housing.


Who We Are

Established in 1970, Serving Seniors helps San Diego County’s low-income seniors overcome poverty through whole-person, wraparound support including meals, housing, health and social services, and enrichment activities. We work at numerous sites across the county and in the homes of hundreds of homebound seniors. Our flagship Gary & Mary West Senior Wellness Center in downtown San Diego is unlike any other senior center in the nation. Bringing together a range of supportive services under one roof, the Senior Wellness Center operates 365 days a year and offers two floors of integrated care, including health and social services, housing navigation, enrichment activities, lifelong learning, a Cyber Café and linkage to over 25 onsite direct service partners, including a senior dental center. Last fiscal year, we provided over 1,400,000 meals and coordinated services to 6,800 older adults.

Requirements

Job Duties and Responsibilities

  • Client Support & Resource Navigation: Provide individualized case management and connect clients to both in-house and external community resources, including health, mental health, and social services.
  • Community Engagement & Outreach: Conduct outreach within supportive housing communities to inform and educate older adults on relevant topics and available services.
  • Program Coordination: Collaborate with the Activities and Volunteer Department to facilitate community-building activities such as cultural celebrations, social events, skill-building workshops, and holiday gatherings.
  • Partnership Development: Build and maintain collaborative relationships with community partners to bring resources to various housing sites.
  • Housing Support: Interface with property management to assist with conflict resolution, tenancy issues, and eviction prevention services.
  • Assessment & Planning: Conduct Universal Senior Assessments and develop care plans to address clients’ unmet needs, including referrals to community-based agencies.
  • Documentation & Compliance: Maintain accurate and timely documentation of client interactions, referrals, and assessments using systems such as CaseWorthy and Microsoft Teams. Adhere to agency policies and budgetary guidelines.
  • Team Collaboration: Participate in interdisciplinary team meetings and represent Serving Seniors at community events and stakeholder meetings.
  • Other Duties: Perform additional duties as assigned to support the overall goals of Serving Seniors.

Requirements


Education and Experience:

  • Bachelor’s degree preferred or equivalent combination of education and experience in social work, gerontology, or human services.
    2+ years of related experience required.

  • High level of professionalism, integrity, and confidentiality.
  • Critical thinking and creative problem-solving abilities.
  • Ability to work independently and collaboratively in a fast-paced, team-oriented environment.
  • Proficiency in Microsoft Office, web-based systems, and client record management software.
  • Excellent organizational and time management skills.

Working Conditions

This position requires the ability to travel between multiple supportive housing sites across San Diego. The employee must be able to remain in a stationary position for extended periods and work in typical office and residential environments. Pay: $23 -$25 DOE
