Independence Plus is now hiring a full-time Clinical Field Supervisor (must be a Registered Nurse) to fill a critical role in our specialized Home Nursing division. This role reports to the Executive Director of Clinical Services and will work out of our Oak Brook, IL (DuPage County) office.


At Independence Plus, we provide skilled at-home healthcare for adult and pediatric patients with a variety of different disease processes and conditions, including spinal cord injuries, traumatic brain injuries, catastrophic injuries and illnesses, ventilator and tracheostomy dependence, neuromuscular diseases, seizure disorders, cardiovascular diseases, and birth or genetic defects.


Job Description Summary


The Clinical Field Supervisor is an exempt administrative position responsible for supervising and evaluating skilled nursing services in accordance with the Physician Plan of Care. 


This role offers a unique opportunity to make a real difference in the lives of medically complex, technology-dependent individuals, while overseeing, guiding, and enhancing clinical programs and services.  


The basic function of this position is to collaborate with the field nurses assigned to each patient under his/her leadership to provide mentoring, modeling, and accountability for the effective scheduling and supervision of the company’s field nurses providing direct care. 


Essential Job Functions/Responsibilities 

  • Directly orients, leads, supervises, trains, and evaluates field nurses assigned to his/her team/s. 
  • Ensure nurses are fully trained and proficient in the electronic medical record, maintaining compliance, data integrity, and security. 
  • Work alongside the administrative team to enhance quality patient care, effective communication, and problem/conflict resolution with referring facility staff, patients, families, and employees. 
  • Reviews patient clinical diagnosis, medications, procedures, and clinical course while establishing long-term therapeutic patient goals. 
  • Collaborates to ensure successful staffing, including nurse orientations of all approved shifts of both current and newly-admitted patients assigned to his/her team/s to determine the best coverage for the patient. 
  • Performs duties surrounding new admissions including but not limited to the initial patient assessment, participate in discharge planning, Plan of Care, discharge/Start of Care, and nurse orientations. 
  • Performs skilled nursing services in accordance with the Plan of Care including supervisory visits, preventative maintenance visits and all routine sterile procedures as required. Utilizes clinical judgement to schedule and perform patient visit assessments during times of illness to prevent and reduce hospital readmissions. 
  • Maintains a connection to frontline challenges by working as field nurse for each assigned patient for a minimum of one (1) shift every six weeks. Works additional shifts as needed for patient coverage.   
  • Participates in a regular rotation with all Home Nursing patients and field nurses for after-hours on-call. 
  • Participates in program evaluation (e.g., Quality Assurance Performance Improvement, Safety/Risk Management, Ethics Committee), licensure and accreditation surveys as requested. 
  • Performs routine documentation audits as required, addresses missing documentation and corrective actions as needed to meet compliance and billing requirements. 
  • Appreciates company vision of providing exemplary care and services for people at home with complex medical and respiratory needs and preventing all complications associated with the four primary organ systems, including skin, chest, bowel, and bladder. 
  • Provides quality customer service and develops collaborative relationships with patients, their families, field nurses, and a multi-disciplinary administrative leadership team. 
  • Maintains and adheres to all accrediting body standards as well as company policies and procedures, State and Federal Regulations, and other payer source requirements. 
  • Accepts accountability for own professional practice and maintains competency by participating in continuing education programs and ensuring all nursing staff complete annual in-services and competencies (ACE).

Requirements

  • Current Registered Nurse (RN) licensure in Illinois. 
  • Minimum of three (3) years of Home Nursing experience or equivalent. 
  • Experience in leading clinical teams using education and motivation, skillful listening and negotiation skills with nurses, patients, and families. 
  • Innovative thinker and collaborative professional who adapt well to changes within an expanding healthcare business.
  • Possess a functional level of proficiency with Microsoft Office Suite and KanTime.
  • Management and conflict resolution experience preferred.
  • Possesses a valid state driver’s license, maintains a good driving record, and reports all violations to direct supervisor and human resources.
