Job Title: Direct Support Professional (DSP)

Reports To: Program Manager

Summary

Supports persons served as a mentor encouraging people to learn and practice skills needed to be successful in their home and community.

Essential Duties and Responsibilities

  • Provide support to persons served.
  • Teach each person served the skills needed to accomplish goals set in their Individual Service Plan (ISP).
  • Document outcomes of ISP.
  • Document facility safety practices, assessments, incident reports and daily logs.
  • Supports persons served in development of connections/relationships in the community.
  • Attend ISP meetings as requested.
  • Attend monthly persons served Self Government meetings.
  • Guide persons served in making responsible choices.
  • Transports persons served as needed.
  • Inform Program Coordinator of issues experienced by person served in ISP implementation.

  • None

Requirements

Minimum Qualifications

High school diploma or equivalent education level.

18 years or older.

Must be able to obtain and maintain certification in CPR/First Aid.

Must be able to obtain and maintain Blood Borne Pathogens, Medication Administration,

Systematic Instruction Strategies, Abuse/Neglect, HIPAA, Confidentiality, and Positive Behavior

training.

Must be able to obtain and maintain Class E driver’s License.

Must be able to obtain and maintain ServSafe Certification (if required).

Must be able to pass a 10-panel drug test.

Must pass a background check including criminal record and driving record.

Ability to perform each essential duty and responsibility.

Excellent verbal and written communication skills.

Exceptional teamwork and problem-solving abilities.

Preferred: Experience working with people with developmental disabilities.


Physical Demands

Prolonged periods of standing, sitting, and walking.

Regularly required to talk or hear.

Ability to use hands and fingers to handle or feel; and reach with hands and arms.

Occasionally required to climb, balance, stoop, kneel, crouch, or crawl.

Occasionally lift and/or move up to 30 pounds; this may be more with certain types of work

assignments.

Specific vision abilities required include close vision, distance vision, peripheral vision, depth

perception, and ability to adjust focus.

essential functions.
