About the Role:
Touchstone Health is looking for a compassionate and connected Community Liaison to grow awareness and access to our Home Health and Hospice services in San Antonio. This is a field-based role, perfect for someone with strong healthcare marketing experience who thrives on building lasting referral relationships and improving care transitions.


Key Responsibilities:

  • Promote the benefits of home health and hospice services to referral partners and the wider community
  • Build and maintain relationships with hospitals, skilled nursing facilities, and physician offices
  • Coordinate referrals and ensure timely, seamless follow-up
  • Identify education opportunities and assist with in-service presentations
  • Collaborate closely with clinical staff to ensure continuity of care
  • Participate in territory planning and help achieve admission goals
  • Analyze referral trends and identify new opportunities for growth
  • Represent Touchstone Health and CareM’s mission with professionalism and heart
  • Maintain HIPAA compliance and uphold ethical marketing practices

Requirements

What We’re Looking For:

  • Proven success in healthcare marketing, preferably in home health or hospice
  • Strong relationship-building skills and a people-first mindset
  • Familiarity with SNF-to-Home transitions is a plus
  • Ability to work independently in a field-based role
  • Highly organized with excellent follow-through
  • Clear, compassionate communicator
  • Motivated by mission, integrity, and community connection
  • Bachelor’s degree preferred
