About the Role:
Touchstone Health is looking for a compassionate and connected Community Liaison to grow awareness and access to our Home Health and Hospice services in San Antonio. This is a field-based role, perfect for someone with strong healthcare marketing experience who thrives on building lasting referral relationships and improving care transitions.
Key Responsibilities:
- Promote the benefits of home health and hospice services to referral partners and the wider community
- Build and maintain relationships with hospitals, skilled nursing facilities, and physician offices
- Coordinate referrals and ensure timely, seamless follow-up
- Identify education opportunities and assist with in-service presentations
- Collaborate closely with clinical staff to ensure continuity of care
- Participate in territory planning and help achieve admission goals
- Analyze referral trends and identify new opportunities for growth
- Represent Touchstone Health and CareM’s mission with professionalism and heart
- Maintain HIPAA compliance and uphold ethical marketing practices
Requirements
What We’re Looking For:
- Proven success in healthcare marketing, preferably in home health or hospice
- Strong relationship-building skills and a people-first mindset
- Familiarity with SNF-to-Home transitions is a plus
- Ability to work independently in a field-based role
- Highly organized with excellent follow-through
- Clear, compassionate communicator
- Motivated by mission, integrity, and community connection
- Bachelor’s degree preferred
