High volume medical group seeking FULL-TIME LPN to provide direct and indirect care for patients. Must have the ability to handle administrative and clinical duties including patient screening and brief assessment of problems, medication review, full vital signs, injections, screening services, and other provider supervised treatments.
Job Description:
Essential functions and basic duties. (This list may not include all the essential functions and duties assigned.)
• Performing Annual Wellness Visits (AWVs)
• Assist in quality and gap improvements program
• Working patient gap outreach reports
• Collaborate with providers and educate patients
• Coordinate of care for patients
• Administer adult and child vaccines
• Refill prescriptions
• Running flu clinics
• Triage phone calls for physician
• Schedule appointments and respond to patient messages and follow through to resolution
• Result call backs
• Verify patient information, records medical history, confirming purpose of visit
• Prepares patients for examination by performing preliminary tests, taking height, weight, blood pressure, respirations, pulse, O2 and, when necessary, HgbA1c, glucose, and urinalysis for acute diagnostic testing or drug screening
• Maintains cleanliness of exam rooms any other workspace provided
• Develop relationships with staff, providers and patients in ways that demonstrate caring and reflects professional behavior
• Performs related work assignments and special projects as assigned in the allotted time frame and reports any delays
• Attends and participates in office meetings as scheduled
• Completes assignments in a timely manner stays informed of all changes in healthcare and shares these changes with practice manager
• Leads and complies with PCMH implementation and standards
• Complies with all HIPAA policies and procedures ensuring confidentiality and patient privacy
• Complies with OSHA/CLIA rules and regulations
• Complies to all company policies and office procedures

SALARY RANGE $31.00 to $32.00 per hour
Requirements
- Graduate of accredited school of professional nursing.
- Have current license to practice as an LPN in the state of CT.
- Demonstrated written, verbal and interpersonal communication skills.
- Knowledge of the CT Vaccine Program a plus
- COVID 19 Vaccination - Preferred but not required.
- Demonstrated written, verbal and interpersonal communication skills.
- Epic and Athena experience a plus. Working knowledge of Microsoft products; Excel, Word.
- at least 2 years or more experience
