About the Role

Description

The Home Health Physical Therapist provides high-quality, patient-centered physical therapy services to individuals in their homes. You will be responsible for evaluating, diagnosing, and treating patients with various physical impairments, functional limitations, and mobility issues. In this role you will create personalized therapy plans, implement therapeutic interventions, and work closely with patients and their families to achieve optimal functional outcomes.


Schedule: Full-Time, Weekdays, Day Shift - 8 hours a day 40 hours a week


Primary Responsibilities and Duties:

  • Conduct thorough evaluations of patients’ physical abilities, limitations, and needs to develop individualized therapy plans.
  • Develop and implement effective treatment programs to improve patients’ mobility, strength, flexibility, and overall functional performance.
  • Utilize therapeutic exercises, manual therapy techniques, modalities, and assistive devices to address specific physical impairments and goals.
  • Educate patients and their families on exercise techniques, home exercise programs and strategies to support recovery and enhance daily living activities.
  • Monitor and document patient progress, responses to therapy, and any changes in condition providing detailed reports to the supervising physicians and other healthcare team members.
  • Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure a coordinated approach to patient care.
  • Provide guidance on the use of assistive devices and home modifications to improve patient functional independence.
  • Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.
  • Participate in the development of care plans and case conferences with the healthcare team to optimize patient outcomes.
  • Tay currently has advancements in physical therapy practices and participates in ongoing professional development.
  • Perform all other duties assigned.

Requirements

Job Qualifications:

  • Valid Physical Therapy license in the state where work is performed.
  • 1 or more years of physical therapy experience, preferably in a home health or similar setting.
  • Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, caregivers, and healthcare professionals.
  • Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment.
  • Ability to build rapport with patients through a compassionate and caring demeanor.

Physical Requirements:

  • Ability to lift and transfer patients, with or without assistance, up to 50lbs.
  • Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods.
  • Strength to handle therapeutic equipment and assist patients with mobility and physical activities.
  • Visual acuity and dexterity for performing tasks such as reading medical records, administering injections, and handling therapeutic tools.
  • Ability to respond swiftly and efficiently to emergency situations.
  • Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces.
  • Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
  • This job requires accepting criticism and dealing calmly and effectively with high-stress situations.
  • The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.
