About the Role

Description

We are seeking a highly skilled and motivated Home Infusion Pharmacist to join our team. As a Home Infusion Pharmacist, you will be responsible for reviewing charts and lab results for an assigned caseload of patients receiving specialized, complex infusion therapy. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care. Job Type: Full-Time, 40 hours per week, Mon-Fri 8:00am-5:30pm CST

Location: Lenexa, KS

Requirements


· B.S. or Doctor of Pharmacy degree is required or Registered pharmacist license if foreign graduate

· Active Kansas pharmacist license in good standing

· Availability Monday through Friday and participation in the on-call rotation. On-call requirements are approximately one weekend/in 3 weeks. The hours of operation are 8:00am-5:30pm CST


Job Responsibilities, included but not limited to:

· Participating in patient evaluation and chart review for acceptance, product selection, device selection and supply management

· Collect, interpret, and evaluate all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness

· Monitor the patient’s progress and assessment results to communicate clinical and technical information with the physician and other members of healthcare team. Most patient contact will be on the phone, and, on occasion, you will meet them in our infusion suite

· Directly supervise pharmacy technicians and support personnel in the appropriate compounding, packaging, and labeling of compounded sterile product

· Assist in training and orientation of new employees, performance improvement activities and educational programs

· Additional licensure may be required in multi-state service areas

· Go in to IV room and mix medications if needed

· Other duties assigned

While performing job duties, the employee is frequently required to communicate verbally and listen attentively. The role necessitates the ability to lift files, open filing cabinets, and bend or stand as needed. Additionally, the position occasionally requires lifting office products and supplies weighing between 20 and 30 pounds.
