General Summary:

Responsible for ensuring the highest standard of patient care at home while providing key oversight and support to our nursing staff. As the highest-ranking clinical professional in the organization, you'll have the unique opportunity to shape and influence the care we provide to our hospice patients and their families.

Reports to: Administrator

Essential Functions:

  • Assist in administering and managing clinical departments, driving productivity and ensuring proper staffing.
  • Collaborate with an interdisciplinary team, including patients, their families, and referring agencies, to provide holistic, patient-centered care.
  • Serve as the clinical leader, providing mentorship, guidance, and support to the nursing staff while maintaining compliance with healthcare regulations.


Participates in the Hospice’s QAPI program. Promotes the Hospice’s philosophy and administrative policies. 

Meets productivity standards. 

Performs on-call responsibilities and provides on-call services to patients and their families as assigned. 

Provides effective communication to patients, their family members, team members, and other health care professionals.


Requirements

Job Qualifications:

Education: 

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing. 

Licensure: 

Current State license as a Registered Nurse; Current driver’s license.

Experience:

Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice.

Skills: 

Nursing skills are defined as generally accepted standards of practice. Knowledge of state and federal regulations.

Good interpersonal skills.

Transportation: 

Reliable transportation and valid auto liability insurance.


Environmental and Working Conditions:

Works in patients’ homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls.


Physical and Mental Effort:

Prolonged standing and walking are required, as is the ability to lift up to 50 lbs and move patients. The position requires working under some stressful conditions to meet deadlines and patient needs, make quick decisions acquire resources, and meet patient/family individualized psychosocial needs. It requires eye-hand coordination and manual dexterity.
