*This position is hired through our Partner Positive Impacts, a social services staffing agency. 


FamilyForward is a non-profit organization dedicated to moving vulnerable children in the direction of hope by delivering comprehensive therapeutic and education services to support biological, foster, and adoptive families. In addition to our work with foster care and adoption, FamilyForward also provides specialized services to prevent and treat abuse and neglect: trauma assessment and psychological evaluation, therapy, coaching and education, therapeutic preschool, and care for children with developmental disabilities.

What to expect on a given day as a Foster Care Case Manager: 

  • You will make decisions related to client services for medical, education, social, and emotional supports; monitor, and ensure clients receive identified, necessary resources for quality services.
  • You will ensure emergency availability is provided to the child, family, and state agency via a rotating on call schedule.
  • You will formulate permanency goals and plans for assigned clients.
  • You will be responsible for preparing records, and documentation of all work with clients, including written assessments, treatment plans, case plans, evaluations, and termination summaries.
  • You will facilitate meetings twice monthly with the biological parents, the child and/or the caregiver in accordance with contract licensing, and or accreditation requirements.
  • You may be asked to attend court hearings by your supervisor. 
  • You will need to Transport clients to and from appointments, Family Support Team (FST) court, school, and moves as needed.
  • You will attend all monthly team meetings, and coordinate /lead Family Support Team (FST) meetings.
  • You will be responsible for identifying, coordinating links, and community resources for clients.
  • As with all positions, this is not an all-inclusive list of duties or projects that you will be asked to perform.

You would be a good fit for this position if you: 

  • Have a bachelor’s degree.
  • Have experience working in foster care and/or with at-risk youth, and families.
  • Have experience providing case management services(preferred).
  • Are a team player.
  • Are committed to innovatively achieving best results for children, and families.
  • Can manage multiple competing demands, make independent decisions, manage time, and resources effectively.
