About Department

This position is part of the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.  The Young Adult Complex Disability service (YACDS) is a state-wide multidisciplinary service for adults aged under 40 with a history of developmental disability, resulting in complex medical needs and physical disability. The key aim of the YACDS support young people and their family transition from paediatric services to adult healthcare services, and improve their health outcomes.

About the Role

You will provide high quality occupational therapy services to the clients of the Young Adult Complex Disability Service. You will work in collaboration with the multidisciplinary YACDS team and other SVHM clinical team’s, as well as external organisations such as the NDIS. You will deliver clinical care within varied settings including; outpatients (currently Fitzroy and Kew locations), in the clients’ own home and their local community.

Classifications for this position will range from Occupational Therapist Grade 2 VF6 – VF9 ($90,235.60 - $103,724.40) per annum, pro rata for part time.

Duties

  • Delivery of accountable, client focused, high quality occupational therapy services, such as; assessment, treatment, evaluation, referral management, reviews, and carer support to all clients within your caseload

  • Drive best practice based on evidence and demonstrated outcomes

  • Work in collaboration with clients, team members, referring professionals, community professional and other key stakeholders

  • Contribute to service and stream development

  • Provide supervision, guidance and support to allied health assistants and undergraduate allied health students

  • Participate in continuous improvement activities within the YACDS

Qualifications, Skills and Experience

  • Bachelor of Occupational Therapy

  • Minimum of four (4) years’ post-graduate experience

  • Eligibility for membership of relevant discipline association

  • Knowledge and experience regarding the delivery of services within the disability sector

  • Highly developed interpersonal skills that incorporate the qualities of excellent communication and leadership

POSITION PURPOSE

  • To work within a flexible, interdisciplinary structure, that ensures the delivery of high quality therapy

  • services to all clients of HIP Young Adults Complex Disability Service (YACDS), making sure that the client’s

  • needs are met and they receive ‘the right treatment in the right place at the right time’.

  • To provide evidenced based, high quality Occupational Therapy services for clients of YACDS, within the

  • YACDs multidisciplinary transition clinics and allied health clinics (postural management, and seating

  • clinics). Clinicians are required to provide assessment and management of clients in a number of settings

  • across the continuum of care within HIP, including client’s residence, community settings, the HIP Centres

  • or Specialist Clinics.

POSITION DUTIES

  • Provide clinical assessment and treatment for all assigned clients as appropriate, according to HIP and St. Vincent’s policies and procedures and relevant service protocols

  • Collaboratively work with the multidisciplinary team members, referring professionals, community professionals and other key stakeholders within a flexible, multidisciplinary structure

  • Complete client goal setting in consultation with patient/family/carers as appropriate. Goals are to be functional and measurable

  • Evaluate treatment / management using the most appropriate methods, making use of objective measures where possible

  • Ensure evidence-based best practice is delivered

  • Maintain current knowledge and skills in clinical best practice and disability and the National Disability Insurance Scheme (NDIS)

  • Supervision of undergraduate allied health students as appropriate

  • Provide guidance, support and direction to Allied Health Assistants as appropriate

  • Contribute to service and stream development as appropriate to the cover period, including ongoing data collection and outcome measures.

  • Participate in relevant in-service programs and meetings. Complete allocated tasks on time

  • Meet activity requirements as agreed upon with Team Leader

  • Maintain accurate statistics and submit in timely manner as per guidelines

  • Maintain the appropriate standard of medical record keeping and data collection as per St Vincent’s and HIP policies

  • Consult with peers and other experts and refer to other healthcare workers when appropriate and in a timely manner

  • Collaborate and clearly communicate with patients/clients and the healthcare team

  • Participate in clinical risk management and continuous quality improvement activities as part of day-to-day work.

Health and Safety

  • Protect the health and safety of self and others, complying with all health and safety related policies, procedures and directions

  • Comply with annual mandatory training requirements

  • Complete required Fire and Emergency Training annually

  • Complete required Workplace Culture and Equity Training annually

  • Attend general hospital orientation within 3 months of commencement

  • As required, comply with fit-testing and PPE requirements

  • Participate in reporting and analysis of safety and quality data including risks or hazards,

  • Report any hazards, near misses and incidents (regardless of whether an injury occurred or not) into Riskman

  • Identify and report any variance to expected standard and minimising the risk of adverse outcomes

SELECTION CRITERIA

ESSENTIAL REGISTRATION, LICENSE OR QUALIFICATION REQUIREMENTS

  •  Bachelor of Occupational Therapy, or other relevant degree

  •  Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)

  •  Eligibility for Australian Occupational Therapy Association membership.

  •  Current Victorian Driver’s License

OTHER ESSENTIAL REQUIREMENTS

  • Minimum of four (4) years post-graduate experience in a relevant field of rehabilitation and or disability and/or community-based service

  • Demonstrated sound knowledge and application of therapy practice and principles particularly in the areas of Disability, Spasticity management, and Neurological Rehabilitation

  • Experience or keen interest in postural management and wheelchair prescription

  • Clinical experience in the area of complex neuromuscular pathologies, particularly cerebral palsy.

  • Knowledge and understanding of the NDIS and other relevant community resources and services

  • Understand and support the principles of HIP continuum of care for clients, particularly in

  • relation to individualized, time-limited, goal-centred episodes of care that aim to improve health outcomes

  • Highly developed interpersonal skills that incorporate excellent communication, organization,

  • planning and problem-solving skills

  • Demonstrated ability to work with minimal supervision, exercise initiative and make decisions independently

  • Demonstrated ability to work in an multidisciplinary team

  • Evidence of ongoing education/professional development relevant to physiotherapy practice

  • A commitment to maintaining and improving occupational health and safety standards

  • Commitment to the values and health care philosophy of St. Vincent’s

  • Competent using Microsoft Office programs, database systems

OTHER NON-ESSENTIAL REQUIREMENTS

  • Previous involvement in quality improvement activities

  • Previous involvement in research projects

  • Experience in service development

  • Demonstrated experience and clinical interest in complex disability, particularly cerebral palsy.
