About the Role

Description

Heartline Staffing, Strategic Talent Partners with Purple Cow Recruiting, is seeking an experienced Director of Case Management for a permanent opportunity in Las Vegas, Nevada.


The Director of Case Management has overall responsibility for managing and coordinating all hospital case management operations. This role ensures compliance with organizational policies and regulatory requirements while leading the integration of patient care coordination and discharge planning processes across hospital departments and external agencies to support continuity of care and optimal resource utilization.


The Director oversees daily operations for hospital-wide case management and is accountable for achieving quality, throughput, and utilization outcomes through strong interdisciplinary collaboration. This position requires a leader who can independently analyze data, develop action plans, and maintain strong strategic relationships with physicians and post-acute community partners.


The Director reports to the VP of Case Management for the Far West Division and provides leadership oversight for Case Management, Social Work, and Chaplain departments. The role supports a high-volume, Level II Trauma facility and may require rotating on-call weekend coverage based on hospital volume and operational needs.


COMPENSATION:

Annual Salary Range:

$135,004 – $180,000 (based on years of experience)


Incentive Bonus:

17.5%


Sign-On Bonus:

$10,000

Requirements

  • Bachelor’s Degree required.
  • Master’s Degree in Nursing, Health Administration, or Business Administration preferred.
  • Active licensure as a Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) required.
  • Minimum of 3 years of acute care hospital case management experience required.
  • Minimum of 2 years of case management leadership experience required.
  • Must have hospital discharge planning experience.
  • Strong strategic leadership skills with ability to independently review data and develop action plans.
  • Must demonstrate strong physician and post-acute community relationships.
