About the Role
<h2>Day Program for Adults with Developmental and Intellectual Disabilities&nbsp;</h2><h2>Licensed Vocational Nurse, LVN</h2><p><a href="https://www.marinventures.org">Marin Ventures</a> is a community day program operating <strong>Weekdays 7am to 3:30 pm</strong> for adults with developmental and intellectual disabilities. A friendly, and inviting place we provide a community for our participants to interact with others and engage in self-directed activities geared to thier interests and abilities. We are known for providing care for medically fragile and in some cases older adults, and for our art program. While some of our clients have medically complex needs, we do not serve individuals with behavioral disabilities.&nbsp;</p><p>The LVN plays a key role in supporting the health and well-being of adults with developmental disabilities attending our adult day program. This is a hands-on role works in conjunction with the Program Director to implement the vision and mission of Marin Ventures. This role holds an assigned caseload of participants with medical needs, ensuring that individuals receive high-quality, person-centered support that promotes independence, personal growth, and community participation. The LVN is also responsible for overseeing all medical service delivery across the organization, tracking and compliance of medications and services for all clients and ensuring that the highest level of adherence to service schedules is maintained with superior client satisfaction. The LVN&nbsp; will be responsible for observing the facility and environment for cleanliness and safety, maintaining supplies and a variety of "on-duty" responsibilities shared by all staff. &nbsp;This individual will have a "do whatever it takes" team attitude jumping in to assist with all clients collaborating closely with participants, their support team members, program management staff, and direct care staff to foster a safe, inclusive, and positive environment. &nbsp;</p><h2>RESPONSIBILITIES</h2><h3>Medical Case Management</h3><ul><li>Manages assigned caseload of participants with identified medical needs. Including proactively planning and training staff to prevent and/or mitigate negative effects of medical conditions.</li><li>Act as the first point of contact to address emergency medical issues such as seizures, injuries, or diabetic emergencies occurring at program.</li><li>Perform routine nursing assessments and maintain accurate health records for all individuals as assigned.</li><li>Develop and update individualized care plans in coordination with program staff, families, other agency staff, and external medical providers.</li><li>Provide direct nursing care as needed and ensure appropriate responses to medical events or emergencies.</li><li>Coordinate with physicians, specialists, and outside service providers to facilitate access to care and ensure continuity of support.</li><li>Supervise, support, and train other team members - direct support professionals (DSPs) on health-related practices, including medication administration, infection control, and emergency protocols</li><li>Ensures compliance with participants&rsquo; medication schedules. Manages centrally stored medication including intake and reordering. Notifies all staff of changes in medications given during the program day.</li><li>Trains DSPs on infection control, Covid-19 protocols, and any other emerging emergency infection control protocols. As well as ensures protocols are being followed by DSPs.<br>&nbsp;</li></ul><h3><strong>GENERAL DSP STAFF DUTIES&nbsp;</strong></h3><ul><li>Ensures compliance with all applicable laws and regulations regarding services for adults with developmental disabilities including reporting suspected abuse or neglect.&nbsp;</li><li>Maintains a safe and clean environment for the program at all times.</li><li>Substitutes on the floor as a DSP when needed.</li><li>Protects and promotes participants&rsquo; rights, including confidentiality, privacy, dignity and freedom from abuse.</li><li>Interacts courteously and appropriately with participants, family members, visitors, staff from other agencies which serve our participants, and other Marin Ventures employees, under a variety of conditions and circumstances.</li><li>Has the ability and desire to learn from and work cooperatively with others.</li><li>Participates in facility committees and task forces as appropriate.</li><li>Follows and ensures all established policies, guidelines and procedures are followed by all in the department, including federal and state regulations, to assure safe practices and quality care of participants.<br>&nbsp;</li></ul><h3>REQUIREMENTS&nbsp;</h3><ul><li>Nursing Licensure: Active and unrestricted LVN license in the state of California.</li><li>Current CPR/FA/AED certification or ability to obtain certification. As well as the ability to satisfactorily complete other required trainings.</li><li>Must have current valid CA driver&rsquo;s license and clean DMV record. Ability to drive agency vehicles.</li><li>Minimum 2 years of LVN experience required.</li><li>Minimum of 2 years of experience working with individuals with developmental disabilities in a community or day program setting is required.</li><li>Minimum of 1 year of supervisory or team lead experience is required.<br><br></li></ul><p>Technical Skills:</p><ul><li>Proficiency with PC and Microsoft Office tools such as Microsoft Outlook, Word, and Excel.</li><li>Experience in electronic records systems/databases, payroll systems, cloud-based software, and basic office applications.<br>&nbsp;</li></ul><h3>PHYSICAL REQUIREMENTS</h3><ul><li>Regular and consistent attendance and the ability to sit and work with hands and arms for at least eight (8) hours per day and five (5) days per week.</li><li>Physical ability to stand, reach, lift, bend, kneel, stoop, climb, walk, crawl, push and pull items weighing up to 30 pounds.</li><li>Hand and finger dexterity for computer, binders, calculator, copy machine, and telephone use.</li><li>Visual skills to include visual acuity and perception.</li><li>Ability to work outdoors in parks and other community settings.
