About the Role

Description

 SUMMARY: 

The physical therapist contracted or employed through the organization is responsible for  providing physical therapy services and for adherence to all conditions outlined in the  Service Employment Agreement. 

ESSENTIAL JOB FUNCTION: Related Responsibilities

 • Provides physical therapy services to patients according to a written individualized  plan of care. This may include, but not be limited to: 

o Assessing and evaluating therapeutic/rehabilitative/functional status, and  participation in the development of the total plan of care. Evaluates home  environment. 

o Assesses for muscle strength, mobility, gait, ROM and transfer capabilities.  Less a goal of rehab, more a focus on maintaining comfort.

 o Directing physical therapy treatment.

 o Provides equipment required to increase patient function and independence.

 o Instructing patients and families/caregivers in the use and care of therapeutic  appliances.

 o Determining priority needs for physical therapy o Reporting to physician and case manager patient's reaction to treatment or  changes in condition.


 • Initiates physical therapy program and instructs other personnel and/or family/  caregivers in certain phases of physical therapy with which they may work with a  patient, as well as instructing them as to the goals of the physical therapy program for  the patient by participating in case conferences.

 • May educate patient in the use of prosthetic device.

  • Prepares and submits clinical and progress summaries based on the attainment of  goals.

 • Participates in patient discharge planning. Prepares and submits a clinical progress  summary based on the attainment of goals directed by organization policy.

 • Provides physical therapy consultation to families/caregivers, as appropriate.

 • Provides in-service education programs for nursing personnel as needed 

• Participates in the interdisciplinary group. 

• Actively participates in the quality assessment performance improvement teams and  activities. 

• Participates in orientation program for home health team members. 

• Provides supervision of physical therapy assistants and home health aides, as  appropriate.

ESSENTIAL JOB FUNCTION: Employee Responsibilities

 • Follows policy and procedures on TB testing.

 • Follows policy and procedures on annual influenza vaccinations.

 • Attends all mandatory in-services 

• Works under the general supervision of the Rehab Team Leader 

Requirements

 REQUIREMENTS: 

Knowledge, skills, and abilities required: 

 • Possesses a degree from a baccalaureate, masters or doctorate program in physical  therapy approved by an accredited organization. Meets personnel qualification stated  in §418.114(b)(7). 

• Licensed to practice as physical therapist within the state. 

• At least two (2) years of appropriate experience as a physical therapist. 

 • Demonstrates good verbal and written communication, and organization skills.

 • Possesses and maintains current CPR certification. 

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. 

• Good oral and written communications 

• Good organizational skills 


PHYSICAL DEMANDS:

Requires heavy physical work more than 70% of the time, including lifting up to 25 pounds  regularly and up to 50 pounds occasionally, standing, pushing, pulling, lifting, walking, and  bending for prolonged periods of time.  WORKING CONDITIONS: • Community home environment  

• Exposure to infectious diseases 

• In and out of automobile 

• Work involves some exposure to unusual elements such as extreme temperatures, fumes,  unpleasant odors, and/or loud noises.  • Work involves frequent exposure to hazards, blood borne pathogens, or physical risks,  which require following basic safety precautions.  

• Normal health care environment.  

• Work involves exposure to plants and animals within the environment.
