WHAT TO BRING:
- Valid Photo ID
OPTIONAL TO BRING: If you are "hired on the spot" it would be beneficial for you to also bring the following items so the hiring process can begin that day.
- Updated resume (if not already submitted)
- State Required License/Certification applicable to the job (CNA, LPN, etc.)
- CPR/BLS/ACLS Card & Infection Control Certificate (For Nursing Only)
- Social Security Card (For W2 Tax Verification Purposes Only)
- I-9 Documentation (CLICK HERE TO VIEW APPROPRIATE I-9 EMPLOYMENT DOCUMENTATION)
- You can provide 1 ITEM from LIST A OR COMBINATION OF 1 ITEM FROM LIST B & 1 ITEM FROM LIST C
- Copy of Insurance Card (If you are eligible for and waiving health benefits because you are covered under a different plan)
- Medical Documentation (5 Items): (1) Physical within the past year (we offer during orientation for free as well). (2) Copy of Immunizations/Proof of MMR Immunity, (3) TB test (Chest X-ray, PPD within the past year, or QuantiFERON Blood test), (4) Proof of COVID vaccine status (if you received), (5) Proof of Flu Vaccine (We can give to you free of charge if you are opting to receive it).
