SUMMARY 

The Housing Case Manager provides case management services to individuals participating in the ESSHI program. These support services include eviction prevention and skill building around tenant responsibilities, tenant advocacy, apartment inspections, and employment/vocational services. Additionally, the case manager will provide instruction in daily living skills enhancement, linkages to appropriate and targeted referral services to include primary care, behavioral health and/or substance use treatment, safety planning, transportation assistance, and support to maintain stable and secure housing. 


ESSENTIAL DUTIES AND RESPONSIBILITIES  

  • Represent agency at community meetings, as assigned.
  • Collaborate with the Outreach/Vocational Case Manager to provide individuals with the support services needed to maintain housing.
  • Meet regularly with individuals on caseload to provide housing care coordination services to support individuals in achieving housing stability, increasing their skills, acquiring employment, obtaining greater self-sufficiency, and decreasing their risk for return or relapse back to homelessness. 
  • Assess clients for vocational, employment and barriers to maintaining secure housing.
  • Develop written housing support plans based upon strengths and individualized needs with individuals on caseload.
  • Assist individuals in acquiring benefits and participating in opportunities that would increase their options and resources, as well as potentially support their transition into gainful employment and/or vocational preparation. 
  • Collaborate with the Sr Case Manager, Assistant Director of Housing and other case managers to ensure continuity of services.
  • Monitor and document client’s progress toward individualized support plan goals.
  • Link individuals to all services and supports necessary to avoid homelessness.
  • Assist individuals with transportation needs.
  • Maintain client confidentiality at all times, including 42 CFR/HIPAA.
  • Report all pertinent information concerning illnesses, accidents, incidents, etc. to supervisor.
  • Perform problem-solving functions to assists individuals in overcoming obstacles.
  • Support individuals when they are in crisis, connecting them with additional services as needed.
  • Maintain electronic health record documentation according to requirements of agency guidelines.
  • Work within program budgets.
  • Monitor and maintain a high standard of service delivery.
  • Follow agency policies and procedures on all program areas.
  • Attend and participate in relevant trainings.
  • Attend staff meetings, supervisions and conferences as assigned.
  • Work closely with the Senior Case Manager and the Assistant Director of Housing to problem solve and be proactive in staying on top of housing policies, procedures and documentation.
  • Assist individuals when they transition to alternative housing of choice arrangements as they become self-reliant. 
  • Performs other duties as assigned or that may develop.

EDUCATION and/or EXPERIENCE 

Required: Associates degree in the Human Services field and two years Human Services experience

Preferred: Bachelor’s degree in Human Services field and one year Human Services experience, particularly with behavioral health, substance use or co-occurring disorder clients. 


CERTIFICATES, LICENSES, REGISTRATIONS

Required: Must maintain valid driver’s license and acceptable driving record.
