About the Role

Description

Position Overview:

Heartline Staffing, strategic partner with Purple Cow Recruiting, is seeking an experienced Primary Care Physician for a value-based care opportunity focused on delivering high-quality, patient-centered care to senior populations.


This physician-led, mission-driven organization is nationally recognized for transforming care delivery for patients with multiple chronic conditions. The model emphasizes quality over volume, strong provider support, and exceptional work-life balance.


Role Highlights:

• 100% outpatient primary care

• Value-based care model

• Average of 18 patients per day

• Small patient panel capped at approximately 450 patients

• Dedicated care team including Medical Assistant and Scheduler

• Administrative time built into the schedule

No nights, weekends, or call

• Monday–Friday schedule

• Clinic hours: 7:30 AM – 5:00 PM (last patient scheduled at 4:15 PM)


Compensation & Partnership:

• Base salary: $250,000

• Bonus: Up to $20,000 in Year 1, increasing upon partnership

• Partnership track with no buy-in

• Typical partnership timeline: 12–18 months

• Approximately 65% achieve partnership within one year

• Partner compensation exceeds $300,000 annually


Patient Population:

• Seniors with an average of 3–5 chronic conditions

• Long-term, relationship-based care model


Additional Practice Support:

• In-house consulting specialists (including Cardiology and Podiatry)

• Holistic services such as Acupuncture, Tai Chi, Yoga, and Nutrition

• Door-to-door transportation provided for patients at no cost

Requirements

• Residency trained in Internal Medicine or Family Medicine REQUIRED

Board Certified in Internal Medicine, Family Medicine, or Geriatric Medicine REQUIRED

• Active and unrestricted medical license required (any U.S. state)

• Relocation candidates accepted

• Commitment to value-based care and senior-focused medicine

• Ability to manage a chronic care population in an outpatient setting

• Spanish language skills preferred but not required
