About the Role

Heartline Staffing is a Strategic Talent Partner with Purple Cow Recruiting. We partner with healthcare organizations nationwide to place experienced clinicians in travel and permanent roles.

Heartline Staffing is seeking an experienced Registered Nurse for a 13-week Home Health travel assignment in Medford, Oregon and surrounding areas. This role supports adult home health patients through skilled nursing visits, care coordination, and comprehensive in-home clinical services.

Assignment Details:

• Schedule: Days, 5x8s (08:00–16:30)

• Start Date: ASAP

• Duration: 13 weeks

• Patient Population: Adult home health patients

• Ratios: 1:1 patient visits (up to 6 visits per day; 30 productivity points standard)

Compensation:

• Gross: $2,646.00/week

• W2: $35.00/hour

• Tax-Free Stipend: $1,246.00/week

Additional Details:

• Guaranteed Hours: Facility may call off up to one shift per two-week pay period

• On-Call: Not required

• Weekends: Not required

• Block Schedules: Not available

• Local Candidates: Accepted

• Transportation: Traveler must have reliable personal transportation; visits may require travel up to 50+ miles

Requirements

Minimum of 1 year of adult skilled home health Registered Nurse experience is required.

BLS certification is required.

Valid Registered Nurse license must be in hand prior to start date.

Experience completing Start of Care assessments is required.

Experience performing Admission Assessments is required.

Experience with Resumption of Care is required.

Experience with Recertification visits is required.

Experience completing Routine Home Health Visits is required.

Experience performing discharge visits, including discharge due to safety or noncompliance, is required.

Experience providing wound care in the home health setting is required.

Experience with medication administration and medication management is required.

Experience following Infection Control Precautions is required.

Computerized documentation experience is required.

OASIS documentation experience is required.

Traveler must have reliable transportation.
