Description

The LPN Medical Records/Infection Control/Staff Education position is a multifaceted role responsible for ensuring adequate nursing staff coverage, maintaining accurate and organized medical records, and providing direct patient care as a Licensed Practical Nurse (LPN) within a skilled nursing facility (SNF) setting. This role requires a strong understanding of nursing principles, excellent organizational skills, attention to detail, and the ability to work effectively in a fast-paced environment.


Schedule: Full-Time, Nights, 10pm - 6:30am


Key Responsibilities:


Licensed Practical Nurse (LPN) Duties

  • Provide direct nursing care to residents as an LPN, including medication administration, wound care, treatments, and vital sign monitoring, as needed to support nursing staff or in emergencies.
  • Assess residents' conditions, report changes to the Registered Nurse (RN) or physician, and document observations accurately.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Educate residents and their families on health maintenance and disease prevention.
  • Adhere to all nursing standards of practice, facility policies, and procedures.
  • Participate in resident care conferences as required.

Medical Records Management

  • Maintain a comprehensive and organized medical records system in compliance with federal, state, and facility regulations (e.g., HIPAA, OBRA).
  • Process new admissions, discharges, and transfers, ensuring all necessary documentation is complete and filed appropriately.
  • Audit medical records for completeness, accuracy, and compliance with documentation standards.
  • Retrieve and distribute medical records as requested by authorized personnel, ensuring confidentiality.
  • Manage record retention and destruction according to established policies.
  • Assist with preparing for surveys and audits by ensuring medical records are readily accessible and organized.
  • Maintain electronic health records (EHR) systems, if applicable, ensuring data integrity and user proficiency.

  Infection Control

  • Develop and enforce infection prevention policies in line with regulations and facility standards.
  • Monitor for outbreaks, investigate cases, and implement immediate corrective actions.
  • Conduct routine audits on hand hygiene, PPE use, isolation procedures, and cleaning practices.
  • Train staff on infection control protocols and proper use of PPE.
  • Collaborate with nursing, environmental services, and leadership to ensure consistent practices.
  • Track and report healthcare-associated infections (HAIs) as required.
  • Maintain accurate records of infections, interventions, and staff compliance.

Staff Education

  • Develop and coordinate ongoing training programs for nursing and clinical staff to ensure competency and compliance with facility policies and regulations.
  • Deliver education on clinical skills, safety protocols, infection control, and new procedures or equipment.
  • Assess staff learning needs and tailor training to address gaps in knowledge or performance.
  • Maintain accurate records of training completion, certifications, and competency evaluations.
  • Collaborate with department leaders to onboard new hires and support their orientation.
  • Promote a culture of continuous learning and professional development.

Requirements

Job Qualifications:

  • Valid Licensed Practical Nurse (LPN) license in the state in which work is performed.
  • 1 year of nursing experience, preferably in a skilled nursing facility, long-term care, or other related fields.
  • Strong clinical skills and the ability to perform a range of nursing procedures with precision.
  • Excellent communication and interpersonal skills, with the ability to interact effectively with residents, families, caregivers, and healthcare professionals.
  • Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment.
  • Ability to build rapport with residents through a compassionate and caring demeanor.

Physical Requirements:

  • Ability to lift and transfer patients, with or without assistance, up to 50lbs.
  • Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods. 
  • Strength to handle medical equipment and assist patients with mobility and physical activities. 
  • Visual acuity and dexterity for performing tasks such as reading medical records, administering injections, and handling medical instruments. 
  • Ability to respond swiftly and efficiently to emergency situations.
  • Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces. 
  • Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
  • Job requires accepting criticism and dealing calmly and effectively with high-stress situations. 
  • The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.
