Description
The Care Coordinator facilitates non-clinical processes in the Care Management program. The Care Coordinator works in collaboration and continuous partnership with the CAIPA Nurse Case Managers, CAIPA Medical Director, Social Workers, members, caregivers, PCPs and SCPs. The Care Coordinator is an integral member of the interdisciplinary team and interfaces with hospitals, Skilled Nursing Facilities, and other Community Based Organizations in a team approach to:
- Promote timely access to appropriate care
- Reduce emergency room utilization and hospital readmission
- Increase comprehension through culturally and linguistically appropriate education
- Ensure continuity of care by managing relationships with tertiary care providers, transitions-in-care and referrals
- Connect patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing healthcare costs
Essential Duties and Responsibilities:
- Perform administrative tasks such as but not limited to document and request discharge summaries from healthcare facilities for patients and/or providers
- Coordinate and facilitate care management activities between patient/caregivers, care managers and their PCP.
- Facilitate linkages to community resources for applicable members.
- Consult and collaborate with other health care providers and specialists to set up patient appointments and treatment plans.
- Complete patient screenings and schedules follow up for case manager to complete comprehensive baseline assessments
- Escalate clinical issues to Nurse or Social Work Case Manager.
- Attend ongoing training and courses to keep abreast of new developments in health care.
- Adheres to organizational standards, guidelines and health care laws and regulations.
- Maintains confidentiality and HIPAA
Requirements
- Medical Assistant with at least 1 year of experience working at a clinic/hospital setting
- Excellent oral and written communication skills and ability to foster a supportive environment for patients.
- Computer skills and proficiency in Microsoft Office.
- Strong analytical thinking and ability to handle multiple tasks concurrently.
- Excellent customer service.
Preferred Skill:
- Bilingual or multilingual fluency: Mandarin/Cantonese
