Make an impact. Join our growing outpatient clinics, focused on improving the wellbeing of families. We are seeking a mission driven and dedicated case manager for our health center in Fullerton.


• Bilingual (English/Spanish)

• Committed to providing high-quality services to underserved patients in Orange County.

• Willingness to work as part of a collaborative team that includes, social worker, physicians, nurse practitioners, and psychiatrist.

• Ambulatory (outpatient) care

• Daytime work schedule - Monday through Friday 8AM - 5PM


Who We Are. We are a collaborative community practice, with locations in Fullerton, Anaheim, Orange, and the High Desert. Our health centers use a team-based model of care that includes family medicine, obstetrics, women’s health, oral and vision care, preventative and educational health, mental health, patient advocacy, case management, and community health. Most of our patients live below 200% of the Federal Poverty level and are best served in Spanish. Essential Functions (What You Do)

  • Help neighbors and health center patients navigate and access community services, health center services, or other applicable services and to adopt healthy behaviors.
  • Promote health education resources.
  • Connect individuals to health insurance coverage and healthcare services.
  • Assess the need for and connect individuals and families to community resources.
  • Assists patients with the completion for forms for available support systems, including applications for disability, rental assistance, SNAP, housing vouchers and temporary cash assistance.
  • Supports Enhanced Care Management efforts by identifying and engaging eligible members, conduct outreach and enrollment.
  • Coordinate care across physical health, behavioral health, and social service systems.
  • Assist patients with scheduling appointments.
  • Perform data entry as needed.

Pay Range: $23.00 to $24.98 per hour

Requirements

Education:

  • High School Diploma or GED

License/ Certifications:

  • CPR/AED / BLS Certification
  • Valid and unrestricted California Driver’s License with clean driving record.

Experience:

  • 1 to 2 years of relevant work experience.

Knowledge/Skills/Abilities:

  • Strong organization and follow-up skills.
  • Ability to multi-task and prioritize.
  • Knowledge of computer software programs and basic office equipment.
  • Demonstrates the ability to read, write and speak English and Spanish without an interpreter.
  • Demonstrates interpersonal/cultural sensitivity and work with a diverse and often underserved population.
  • Effective written and verbal communication across all levels of the organization and with vendors.
  • Highly organized with strong time management skills, being able to perform simultaneous tasks, prioritize them effectively and follow through quickly.
  • Proficiency with EMR systems.
  • Communication skills consistent with the facilitation of health and wellness topics to individuals, small and large groups of community residents.
  • Reliable vehicular transportation to travel to community events.

Preferred Requirements


Education:

  • Coursework or training related to care coordination, behavioral health, or community health.

Experience:

  • At least one year of experience working in community-based care settings, care coordination, or programs serving high-need populations.

Knowledge/Skills/Abilities:

  • Previous Medical Receptionist or Medical Assistant experience.
  • Prior experience utilizing Epic electronic health record system.
  • 1-2 years performing Enhanced Healthcare Management
