About the Role

Description

The Care Coordinator is responsible for the coordination of patient care through multidisciplinary collaboration with medical, nursing and ancillary departments.  The Care Coordinator assists with the planning of patient care including, but not limited to, discharge planning.  The Care Coordinator is responsible for assuring effective use of available hospital and community resources and monitoring patient outcomes through concurrent utilization review and proactive discharge planning.  Has full access to patient health information.


Shift: M-F, 8:00 AM - 5:00 PM, weekend rotation, some call

Requirements

  • Currently licensed as a Registered Nurse by the North Carolina Board of Nursing. BSN preferred.
  • BLS certification through American Red Cross or American Heart Association
  • Minimum of 2 years of clinical experience in a hospital setting.
  • Previous utilization and/or discharge planning experience preferred. 
  • Excellent organizational and communication skills required.
  • Must possess full range of body motion to pass a basic FIT test to include bending, stooping, standing and sitting for extended periods of time.  Must be able to lift and carry up to 30 pounds.
