The Mobile Outreach Coordinator (MOC) is responsible for independently planning, coordinating, and operating mobile medical outreach services in the community. This role requires a highly self-motivated, organized individual who can manage both the administrative and field-based operations of a mobile medical unit while ensuring compliance with New York State, federal, and FQHC regulations.
The MOC serves as the primary point of coordination for mobile clinic activities, including scheduling, patient intake, site scouting, relationship-building with community partners, and day-to-day mobile unit operations. The position plays a critical role in expanding access to care by bringing services directly to underserved populations and ensuring seamless connection to in-office services.
Essential Job Responsibilities
Mobile Unit Operations & Compliance
- Independently operate and coordinate the mobile medical unit as a compliant mobile medical site under FQHC, NYS, and federal guidelines
- Ensure mobile unit readiness, including equipment, supplies, connectivity, privacy, and infection control standards
- Maintain compliance with organizational policies, clinical protocols, HRSA requirements, and applicable state and federal regulations
- Coordinate transportation, maintenance, stocking, and scheduling of the mobile unit
- Monitor mobile operations and escalate operational or compliance concerns as needed
Outreach & Site Development
- Actively scout, assess, and establish new community locations for mobile outreach services
- Develop and maintain Memorandums of Understanding (MOUs) with community organizations and host sites
- Build and sustain strong relationships with community agencies, local partners, and potential patients
- Represent the organization at community events, outreach activities, and partner meetings
Patient Engagement & Administrative Support
- Coordinate and schedule patient appointments for mobile visits
- Complete, collect, and file new patient paperwork in accordance with organizational and regulatory requirements
- Support patient intake processes, including consent, eligibility screening, and documentation
- Assist patients with navigation of services and linkage to in-office care as appropriate
Program Coordination & Performance
- Support development and tracking of mobile outreach goals, metrics, and utilization
- Maintain accurate records, reports, and documentation related to mobile outreach activities
- Collaborate with clinical, administrative, and outreach teams to improve service delivery and access to care
- Participate in continuous quality improvement activities related to mobile services
Requirements
Skills & Qualifications
- Highly independent, self-motivated, and organized with strong problem-solving skills
- Strong administrative skills, including scheduling, documentation, and record management
- Excellent interpersonal and relationship-building skills with diverse communities and partner agencies
- Working knowledge of community outreach, mobile health services, or healthcare operations
- Digital literacy, including Microsoft Office, electronic health records, email, and web-based systems
- Ability to work flexible hours, including evenings and occasional weekends
- Bilingual (Spanish/English) preferred
Education & Experience
- Experience working in healthcare, public health, community outreach, or human services
- Experience with mobile medical services, FQHCs, or community-based programs preferred
- Associate’s or Bachelor’s Degree in health, human services, or related field preferred
- Valid New York State driver’s license required
