About the Role

Coordinates care for Breast Cancer patients working with the physicians and multidisciplinary team.


Principal Accountabilities:


1. Serves as patient and family advocate. 
2. Assists patients with scheduling of appointments to expedite the plan of care and to promote continuity of care. 
4. Assists in education of the patient/family to understand Breast disease process, care map and treatments.
5. Assists in answering patient calls, initiate call backs and follow-up to meet the needs of the patient. 
7. Supports a smooth transition of patients from outside referrals to internal referrals for appropriate treatment. Assists with Breast MDCs.

8. Identifies Barriers to Care and refers to support staff. Participates in Breast Cancer Support Group.

9. Provides Treatment Summaries' and Survivorship Plans with the physician.

10. Participates in Breast Programs and community activities.
11. Performs other related duties as assigned.
