About the Role

Description

  

The Supportive Services Coordinator is responsible for coordinating social services, supportive case management, and resident engagement programming to meet the diverse needs of the JFS Housing Community's resident population. This position works with individuals and families who may be at higher risk for social, housing, or life stability challenges, and provides supportive services such as assessment, crisis response, advocacy, and care coordination. The Supportive Services Coordinator also develops and facilitates inclusive social, educational, recreational, and wellness-focused programming that promotes community connection, stability, and independent living. The position requires strong communication, collaboration, and problem-solving skills to work effectively with residents, families, staff, and community partners. This role supports an inclusive, campus-wide service model that connects and serves residents across all JFS Housing buildings. This role reports to the Community Supportive Services Supervisor and works closely with the Director of Assisted Living. 


Responsibilities and Duties:


Supportive Services/Case Management

  • Conduct comprehensive assessments of residents’ needs, strengths, and barriers using a brokerage case management model.
  • Provide supportive services and case management to residents who may experience social, housing, or life-stability challenges.
  • Respond to residents experiencing heightened stress, emotional distress, or situational crises, utilizing problem-solving, de-escalation, and referral to appropriate supports. 
  • Coordinate and link residents to services that promote stability, independence, and well-being, including housing, medical and mental health care, employment, benefits, and other social services.
  • Conduct home visits as needed for assessment and ongoing case management.
  • Maintain accurate, timely, and confidential client records and statistical data in compliance with agency and State regulations.
  • Collaborate with Community Case Management, Assisted Living staff, property management, family members, and external providers to ensure continuity of care.


Programming and Resident Engagement

  • Develop, coordinate, and facilitate cognitive, creative, physical, social, recreational, and wellness-focused programming for residents across the JFS Housing Community.
  • Build relationships with residents to understand interests, abilities, and needs and design programming accordingly.
  • Develop and distribute monthly programming calendars, flyers, and outreach materials.
  • Update and maintain bulletin boards and signage with information about programming and community resources.
  • Actively engage in and lead on-site and off-site activities and outings.
  • Arrange transportation for activities and drive agency vehicles as required. 
  • Identify, requisition, and manage supplies needed for planned activities.
  • Train, support, and oversee staff and volunteers assisting with programming.
  • Maintain records and data to track participation and programming outcomes.

Requirements

  Qualifications

  • Bachelor’s degree in Social Work or related human services field.
  • Experience working with diverse or vulnerable populations, including individuals or families facing social, housing, or life-stability challenges. Experience working with older adults 
  • Demonstrated ability to provide supportive services, crisis response, and resource coordination.
  • Strong communication, collaboration, and problem-solving skills.


Preferred Qualifications

  • Experience working with older adults and/or individuals aging in place in assisted living or supportive housing settings, including support related to mental health, cognitive changes, and chronic health conditions. 
  • Familiarity with aging-related service systems and community resources, including Medicare, Medicaid, long-term care programs, benefits navigation, and collaboration with families, caregivers, and interdisciplinary care teams.


General

  • Must have a valid driver’s license, access to reliable vehicle, and automobile insurance coverage throughout employment.
  • Knowledge, understanding, and/or willingness to learn Jewish culture and traditions and those of the Agency population as necessary.
  • Accept other duties and responsibilities as assigned.
  • Candidates must successfully pass a criminal background check with the Wisconsin Department of Justice and a caregiver background check with the Wisconsin Department of Health Services, the Department of Children and Families and the Department Safety and Professional Services. 
  • Candidates must pass a Dru Sjodin National Sex Offender and Wisconsin Sex Offender Registry screening
