Description

Helping Hand is seeking a dedicated and compassionate Case Manager/QIDP to join our Case Management Team. In this role, you will serve as a key advocate, communicator, and coordinator for the individuals we support, ensuring each person's goals, rights, and needs are honored. You will work closely with individuals, families, providers, and interdisciplinary teams to deliver high-quality, person-centered care.



SCHEDULE AND COMPENSATION:

  • On-site position, Monday-Friday
  • 40 hours per week. 
  • Some flexibility is needed during varying hours including evening and weekends in accordance with the position’s needs.
  • Salary range $55,000-$58,000/year (exempt).  
  • Salary based on skills and experience.
  • Up to 8% annual bonus potential.
  • 403b retirement plan with up to 4% company match.

WHAT YOU'LL DO:

  • Advocate for and protect the rights of individuals served.
  • Coordinate care and services across providers, families, and support teams.
  • Develop, monitor, and document individualized service plans.
  • Facilitate team meetings and ensure continuity of care.
  • Support clients in accessing benefits, healthcare, employment, and community integration.
  • Conduct home visits, provide direct support, and ensure services meet compliance standards.
  • Other duties as assigned.

Requirements

WHAT YOU BRING:

Education:

  • Bachelor's degree in Psychology, Social Work, or a related field required.
  • QIDP certification (preferred).

Experience: 

  • One (1) year of experience working with individuals with intellectual and developmental disabilities.

Skill Sets:

  • Strong organizational, communication, and problem-solving skills.
  • Proficiency with Microsoft Office, Teams, Zoom, etc.
  • Valid driver’s license with an acceptable driving record, ongoing personal motor vehicle insurance, and access to a reliable vehicle that can be used for work-related duties as required. 
  • Ability to pass PACE certification requirements (preferred).
  • Ability to self-transport to work and home visits
  • Ability to promote and adhere to company values.

WORK ENVIRONMENT:

  • The person in this position may encounter clients exhibiting physical aggression; must be comfortable working in periods of crisis and feel comfortable de-escalating individually or as a team.
  • The person in this position needs to occasionally move about inside the office to assist clients, access file cabinets, office machinery, etc. 
  • The ability to observe details at a close range (within a few feet of the observer). 
  • Consistently position self to maintain files in cabinets or assist clients. 
  • The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in all situations.
