About the Role

Greater Nashua Mental Health is seeking a compassionate, community-focused professional to join our team. In this role, you’ll provide outreach, education, and support to individuals experiencing homelessness or at risk of homelessness who are living with mental illness or co-occurring conditions. You’ll also build strong partnerships with local agencies to improve access to treatment, services, and housing.


What You’ll Do

  • Conduct outreach and provide case management support in the community
  • Engage individuals in mental health treatment and supportive services
  • Collaborate with shelters, community agencies, landlords, and local partners
  • Deliver trainings and education on mental health, de-escalation, and resources
  • Document services and outcomes in HMIS and other required systems

What You Bring

  • Bachelor’s degree in social work, Psychology, or related human services field (Master’s preferred)
  • 3–5 years of experience working with individuals experiencing homelessness and/or serious mental illness
  • Strong communication, outreach and collaboration skills
  • Valid driver’s license and comfort working in the community
