Job Type: Full-Time (Part-Time/PRN considered)

About the role

Sylva Valley Health & Rehab is hiring an Occupational Therapist (OT) and/or Occupational Therapy Assistant (OTA) to provide high-quality therapy services in a skilled nursing setting. This role is ideal for clinicians who are passionate about helping residents improve independence, safety, and quality of life through meaningful, functional goals.

Key responsibilities (OT)

  • Evaluate residents and develop individualized occupational therapy plan of care
     
  • Provide skilled OT interventions focused on ADLs, transfers, safety, cognition, and adaptive equipment
     
  • Document evaluations, progress notes, and outcomes to support skilled coverage and care needs
     
  • Coordinate with PT/SLP, nursing, providers, and the interdisciplinary team
     
  • Educate residents/families on safety strategies, home programs, and equipment
     
  • Participate in care conferences/IDT and discharge planning as needed
     

Key responsibilities (OTA)

  • Provide OT treatment interventions under the supervision of an OT
     
  • Document treatments and resident response/progress
     
  • Communicate changes in condition to the supervising OT and care team
     
  • Assist with resident/family education per plan of care
     
  • Support discharge planning and continuity of care
     

Requirements

  • OT: Current Occupational Therapist license + graduate of an accredited OT program
     
  • OTA: Current Occupational Therapy Assistant license + graduate of an accredited OTA program
     
  • Strong documentation and communication skills
     
  • SNF/rehab experience preferred (new grads considered)
