Job Type: Full-Time (Part-Time/PRN considered)

About the role

Sylva Valley Health & Rehab is hiring a Physical Therapist (PT) and/or Physical Therapist Assistant (PTA) to provide high-quality rehab services in a skilled nursing setting. This role is ideal for clinicians who value patient-centered care, strong clinical outcomes, and teamwork with nursing and the interdisciplinary team.

Key responsibilities (PT)

  • Evaluate residents and develop individualized plan of care
     
  • Provide PT treatment to improve mobility, strength, balance, gait, and safety
     
  • Document thoroughly to support skilled coverage and clinical progress
     
  • Coordinate with OT, SLP, nursing, and providers regarding changes in condition
     
  • Educate residents and families on exercises, safety, and discharge planning
     
  • Participate in care conferences/IDT as needed
     

Key responsibilities (PTA)

  • Provide treatment interventions under the supervision of a PT
     
  • Document treatments and resident response/progress
     
  • Communicate changes in condition to the supervising PT and care team
     
  • Assist with resident and family education per plan of care
     
  • Support discharge planning and continuity of care
     

Requirements

  • PT: Current Physical Therapist license + graduate of an accredited PT program
     
  • PTA: Current Physical Therapist Assistant license + graduate of an accredited PTA program
     
  • Strong documentation and communication skills
     
  • SNF/rehab experience preferred (new grads considered)
