  • Work across the Outer South and Mornington Peninsula areas + Hybrid working conditions.
  • Competitive salary starting from $51.23 depending on experience + Super + generous Salary Packaging benefits of up to $18,550      

The role
Each is seeking an experienced and people‑focused Team Leader to support and guide our Care Finder team, helping older Australians who may face vulnerability, isolation, or barriers when accessing aged care and community supports.

If you’re a natural leader with strong sector knowledge and a commitment to person‑centred, dignity‑driven practice, this role offers the opportunity to lead a passionate team while still remaining connected to clients through a 50% caseload.

As a Team Leader in the Care Finder program, you will:

  • Provide leadership, supervision and day‑to‑day support to the Care Finder team.
  • Coordinate workloads, oversee service delivery and ensure quality practice and program guidelines are upheld.
  • Assist in developing, implementing and evaluating a robust and responsive service model.
  • Facilitate team meetings, staff supervision and induction for new team members.
  • Carry a 50% caseload, providing specialist, intensive assistance to older people needing help to understand and access aged care and community supports. 
  • Support clients to navigate My Aged Care, assessments and service referrals using warm, supported pathways.
  • Build and strengthen relationships across government, community and voluntary services to promote the program and reach vulnerable populations.
  • Maintain accurate case notes and contribute to reporting processes, reflective practice and ongoing professional development.

Who we are looking for
You will thrive in this role if you bring:

  • Demonstrated experience leading and managing a team, ideally within aged care, community services or a related sector.
  • Strong communication skills, empathy and the ability to build rapport with older adults and their families.
  • Highly developed organisational, time‑management and problem‑solving skills.
  • Ability to engage and negotiate with a wide range of stakeholders. 
  • A solid understanding of aged care supports, referral pathways, and the policy environment in the community services sector.
  • Commitment to cultural safety, trauma‑informed practice and person‑centred care. 
  • The ability to work both independently and as part of a collaborative team.
  • A second language is highly desirable.

Qualifications

  • Relevant qualification (Social Work, Human Services, Aged Care, Community Services, Health or related).
  • Current Australian driver’s licence and access to an insured vehicle.
