About the role
Every person deserves a safe place to call home. As a Case Manager, you’ll walk alongside those doing it tough, helping them build a path out of homelessness through person-centred, trauma-informed support. You will work flexibly and creatively with people in crisis – listening deeply, planning collaboratively, and acting with empathy and determination with individuals and families at risk of or experiencing homelessness across Griffith and the wider Murrumbidgee region
This is a full-time permanent role based in Griffith. Salary is in accordance with SCHADS Crisis Level 2.
How you will make an impact
Provide tailored case management to individuals and families experiencing housing vulnerability, including crisis, transitional and outreach support
Build strong, respectful relationships that uphold dignity, choice, and cultural safety
Develop client-led case plans that support housing stability, wellbeing, and independence
Work closely with health providers, real estate agents, housing agencies, and community partners to build sustainable outcomes
Travel regularly to rural and remote areas to meet people where they are
Contribute to a collaborative team culture that values learning, integrity, and innovation
Advocate fiercely for your clients while maintaining professional, trauma-informed care standards
Keep accurate, confidential client records and data in line with SHS requirements
What you will bring
A minimum Diploma in Community Services, Social Work or related field (or working toward a relevant degree)
At least 2 years’ experience in the community sector, ideally supporting clients with complex needs
A solid understanding of trauma-informed care, homelessness, mental health, and AOD issues
Strong cultural awareness and a commitment to inclusive, non-judgemental support
A valid NSW Working With Children Check
