SUMMARY

The TCM will assist people receiving services in sustaining recovery and gaining access to needed medical, social, legal, educational, housing, vocational and other services and supports. The TCM’s primary focus is on individuals with complex or serious mental health, substance use and co-occurring conditions. TCM will support individuals during critical periods, such as acute episodes, care transition and navigating complex healthcare and social services systems. This also extends to supporting people with high risk due to health-related social needs, suicide or overdose and transitioning from various levels of care.


ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Support and assist the Director and designees with relevant program responsibilities related to program and department needs, Narcan distribution, value-based initiatives, and attend trainings, meetings, community events and cross trainings as beneficial.??
  • Act as the liaison between members of multidisciplinary teams in efforts to support increased patient engagement and adherence to appropriate and person-centered treatment.??
  • Convey information inter/intra agency; communicate professionally with team regarding changes and conflicts.?
  • Develop relationships with community stakeholders to enhance overall awareness, adopt harm reduction strategies, reduce stigma, facilitate efficient client care and prevent overdose deaths in the tri-county communities.?
  • In collaboration with the medical and clinical staff, use standardized assessment tools, engagement process, and harm reduction methods to offer patients services that address behaviors, mental health, substance use and/or co-occurring mental health and substance use that put them at risk for recurrence, suicide, self harm and/or overdose.? Document all efforts in the clinical record.?
  • Must maintain all laws and policies concerning corporate compliance, confidentiality, security, and client rights as specified in the employee manual, 42 CFR Part 2, and HIPAA.
  • Exercise problem-solving and conflict resolution.?

Targeted Case Manager primary work responsibilities:

  • Maintain a caseload and performs primary case work responsibilities
  • Identify and implement appropriate interventions to provide timely, high-quality and efficient care.
  • Utilizing motivational interviewing, strengths-based, culturally appropriate and person centered practices to develop new strategies on capturing and addressing barriers to services.
  • Scheduling and meeting with clients at the clinic or community setting and assisting clients with obtaining housing, food stamps, transportation, insurance or other immediate and crisis needs
  • Educate and discuss support and wrap around services at the clinic, home and community
  • Collaborating with care team members to provide appropriate treatment, referrals and goal development for clients.
  • Maintain working knowledge of patients’ progress throughout the life cycle of treatment.
  • Collect and organize patient information to clearly evaluate strengths and barriers to identify goals, such as pulling reports from Healtheconnections and Psykeys
  • Scheduling targeted case management appointment, tracking follow up

NYS Certification for Opioid Overdose Prevention Program

  • Must be or become certified NYS OOPP train the trainer.
  • Support current OOPP distribution sites and identify applicable opportunities for distribution site expansion with leadership approval.?
  • Conduct or coordinate initial and refresher train-the trainer session for overdose trainers and responders.
  • Provide supervision of approved trainers at least 4 times a year by phone or in person (with 100% participation by all partner agencies/distribution sites).
  • Track and Report all requested and necessary OOPP data

EDUCATION and/or EXPERIENCE

Required: High school diploma or equivalent. Must have knowledge and understanding of working with populations with mental health, substance use and co-occurring disorders, children and youth diagnosed with severe emotional disorders.


Preferred: at least two years’ experience.


An equivalent combination of training, education, and experience that would provide the required knowledge and abilities would be considered.


CERTIFICATES, LICENSES, REGISTRATIONS

Must maintain valid driver’s license and acceptable driving record.
