YOUR TOP RESPONSIBILITIES
- You will be responsible for conducting case management assessments on community, shelter and program clients developing individual service plans, and assisting for proper program placement.
- You will coordinate the delivery of support services for clients such as medical, legal and ancillary to remove potential obstacles and enhance the potential for client success.
- You will develops and maintain relationships with outside agencies to which clients can be referred.
- You will monitor client progress towards achieving service plan goals, conducts regular plan reviews and adjusts as necessary.
- You will maintain records of medical appointments, court appearances, or other appointments as required by program procedure and policy.
- You will use the PRM database data, case conference and other methods to identify client needs and appropriate interventions and ensure the quality of client data in PRM database.
- You will sets case management appointments with clients who are enrolled in a PRM program.
- You will be responsible for driving clients to appointments.
- You will lead or participate in devotions, prayers, or other organization activities that promote a credible Christian witness and lifestyle.
Requirements
WHAT YOU SHOULD HAVE
- Bachelor's degree in social work, psychology, counseling, social services or a related field or four years of experience in case management, social work, direct service, or behavioral health or an equivalent combination of education and experience required in performing case management in similar settings is required.
- Fluency speaking Spanish is preferred.
- You have experience working with people who are in recovery and who have been through trauma.
- You properly exercise discretion.
- You have a command of all Microsoft Office applications and experience with databases.
- You have a solid and credible Christian witness and lifestyle that exemplifies Christ in character and actions.
