Description
The Physical Therapist provides professional physical therapy services in a Critical Access Hospital (CAH) setting including acute care, outpatient services, geriatrics, swing bed, long-term care, and wound care in accordance with the Physical Therapy Practice Act and organizational standards.
Additional services may include ergonomic assessments, functional agility testing, pre-employment screenings, and staff and community education.
The Physical Therapist collaborates with providers and interdisciplinary team members to deliver high-quality, patient-centered care while maintaining professional standards and regulatory compliance.
ESSENTIAL DUTIES & RESPONSIBILITIES:
· Evaluates patients, establishes appropriate treatment goals, and delivers skilled physical therapy assessments and interventions as prescribed by providers.
· Develops, implements, and modifies individualized treatment plans based on patient needs and progress.
· Reassesses patient progress regularly and revises treatment plans and goals as appropriate.
· Discharges patients when therapy services are no longer medically necessary or beneficial, ensuring appropriate follow-up recommendations.
· Delegates physical therapy assistants in accordance with state practice guidelines and organizational policy.
· Collaborates effectively with physicians, physician assistants, nurse practitioners, nursing staff, and other clinical team members to enhance patient outcomes.
· Provides services across care settings including acute inpatient, outpatient, swing bed, and long-term care.
· Performs wound care management within scope of practice and competency, as applicable.
· Completes accurate, thorough, and timely documentation for all patient encounters, including evaluations, reassessments, daily notes, plan of care updates, and discharge summaries.
· Participates in quality improvement initiatives and supports departmental goals and performance standards.
· Performs pre-employment assessments as assigned, including functional agility testing, lifting assessments, and ergonomic evaluations.
· Assists in orientation and training of new staff and students as requested.
· Demonstrates positive, supportive, and professional communication with patients/residents, families, visitors, and staff.
· Maintains current licensure and assumes responsibility for professional development, meeting all CAH and departmental continuing education requirements.
Requirements
Training and Qualifications:
- Graduate of a CAPTE-accredited Physical Therapy program (DPT) or equivalent entry-level degree
- Current MT Physical Therapist license
- Current BLS certification
- Ability to communicate effectively and diplomatically within a multi-functional team
- Strong organizational skills and attention to detail
- Ability to successfully function in a fast paced, service oriented environment
- Experience in understanding and usage of computers as well as the ability to learn applications relevant to the position
