SUMMARY OF POSITION:
The RN Health Care Supervisor reports to the V.P. of Health Services. The RN Supervisor oversees all health-care services provided to the members of the Day Habilitation as per CMR 419.24. The HCS will be responsible for the supervision of Licensed Practical nurses over multiple sites. This is a salary, exempt position.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Coordinate medical services with each member’s primary physician or medical clinic.
• Oversee all health-care services provided to the members at each program.
• Participate in the continuing interdisciplinary evaluation and review of each member's needs and status.
• Obtain reports and approval of treatment plans from primary physicians.
• Ensure and safeguard Venture consumers' human dignity, rights and liberties.
• Document in each member's record any recommendation resulting from the periodic reviews; and advise the program director and the day habilitation staff of any medical problems that may hinder a member's progress or that may be aggravated by activities at the program.
• Provide direct supervision to each LPN nurse for a minimum of six hours per week for 28 participants and an additional 2 hours per 14 participants.
• Management of medication as per CMR 419.431.
• Conduct medication reviews during case reviews a minimum of every three (3) months, or more frequently on an as needed basis.
• Monitor and record vital signs and weights every six (6) months, or on an as needed basis.
• Contact family/community residences, physicians, other health care professionals and service coordinators as necessary regarding specific consumer medical issues.
• Participate in DHSP meetings as necessary and assist and cooperate in developing DHSP goals for each consumer and/or develop medical status assessments.
• Act as a member of the Interdisciplinary Team.
• Screen admission applications for medical information prior to Interdisciplinary Team Review.
• Monitor the general health and well-being of each consumer on a daily basis and communicate any concerns or issues to Administrator/Program Director.
• Obtain and maintain medical records including yearly update of emergency fact sheet and authorization for emergency medical care.
• Develop and maintain nursing care plans.
• Document nursing notes as needed on monthly and quarterly progress notes.
• Maintain consumer file.
• Adhere to all medical and funding source regulations and Venture policies and procedures.
• Adhere to all medical requirements from accreditation agency.
• Education to consumers in hygiene and health concerns.
• Oversight of maintenance-therapy treatment as recommended by a Therapist
• Provide and coordinate for medical/health related in-service training to employees, including, but not limited to: Universal Precautions, Venture's Bloodborne Pathogen Exposure Control Plan.
• Acquire and maintain certification to become an agency MAP trainer.
• Availability to provide at least two agency MAP training sessions per year and participate in staff MAP recertification testing
MARGINAL DUTIES AND RESPONSIBILITIES:
• Attend and participate in meetings, trainings and development.
• Actively participate on work teams to solve problems, share ideas and plan for change.
• Develop and expand opportunities for community integration to address consumers' employment, social and recreational interests, using agency and one's own network of contacts.
• Maintain confidentiality and positive demeanor while successfully juggling multiple priorities.
• Provide recommendations to your supervisor regarding ways to improve the current systems
• Maintain CPR and AED certifications
• Other duties as assigned.
PHYSICAL AND MENTAL REQUIREMENTS:
·• Ability to establish rapport with the disabled population, their families, co-workers and service providers.
• Maintain trainings specific to individual programmatic issues.
• Ability to plan and organize your work and the activities of others.
• Actions should reflect judgment that would not jeopardize the health and safety of the individuals.
• Proficient in Microsoft Office and a working knowledge of office equipment.
• Effective verbal and written communication skills
• Strong interpersonal and communication skills, and the ability to work with a wide range of employees.
• Must be able to bend, lift, stoop, reach and file.
Requirements
MINIMUM ACCEPTABLE QUALIFICATIONS:
· Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.
· At least one year of full-time working experience as an RN.
· Must be 21+ years old and have a valid driver’s license. Daily use of a car on site and be available by telephone for daily contact is also required.
· Excellent knowledge of MAP and DDS regulations preferred
