THE POSITION AND JOB SUMMARY
ACRC is seeking a Service Coordinator in the Enhanced Service Coordination Unit in our Sacramento office. This position will provide enhanced case management and advocacy for children and adults with developmental disabilities within the African American community. For purposes of this caseload ratio, clients with low to no purchase of service expenditures may include individuals who reside independently in their community or with family. The position will work to reduce disparities and improve equity in purchase of services, identify and resolve barriers to accessing services and supports; enhance the relationship between consumer/family and ACRC; empower individuals to partner with ACRC and other community agencies and assist with identification of new resources that are culturally relevant to the client. These are specialized caseloads that require specialized language and culture accessibility. The position works in collaboration with ACRC’s Cultural Diversity Specialist and other Departments within ACRC to identify innovative approaches to service delivery. This position requires extensive travel as the unit serves all 10 counties: Alpine, Colusa, El Dorado, Nevada, Placer, Sacramento, Sierra, Sutter, Yolo, and Yuba counties.
