Join our team as a Blended Case Management (BCM) Supervisor, where you’ll guide dedicated case management staff, support individuals living with serious and persistent mental illness, and help shape high-quality, person-centered services across the community.
The BCM Supervisor oversees day-to-day clinical and operational functions of the Blended Case Management program in Beaver County under the direction of the Program Director. This role ensures quality care delivery, regulatory compliance, staff development, and strong collaboration with community partners to support meaningful recovery outcomes for clients.
Starting salary of $48,000 annually
Location: 1598 Virginia Avenue, Monaca, PA 15061
Position Details
- Provide clinical and operational oversight of the Blended Case Management (BCM) program, ensuring high-quality, community-based care for individuals with serious and persistent mental illness.
- Supervise, coach, and support BCM staff through weekly clinical supervision, crisis guidance, strengths-based problem solving, and participation in hiring and onboarding.
- Lead service planning, admissions, referrals, discharge coordination, and treatment team meetings to promote continuity of care and positive client outcomes.
- Monitor documentation, productivity, and compliance to meet county, managed care, and state requirements while assisting with required program reports.
- Serve as a liaison with families, providers, and community partners, representing program operations and fostering collaborative, recovery-oriented services.

The ideal candidate will possess the following qualifications:
- Bachelor’s degree in sociology, social work, psychology, gerontology, anthropology, history, criminal justice, theology, counseling, or education and five (5) years of direct mental health care experience, including two (2) years in a supervisory role or
- Registered Nurse (RN) and five (5) years of direct mental health care experience, including two (2) years in a supervisory role or
- bachelor’s degree in nursing with three (3) years of direct mental health care experience
