Job Description:

MHCGM is seeking an enthusiastic Team Coordinator to oversee the dynamic Clinical Case Management (CCM) team! The CCM team provides recovery-oriented services to patients with a wide range of diagnoses and clinical presentations including thought disorders, personality disorders, mood disorders, and more. This energetic team believes that everyone deserves a life worth living and supports all pathways to recovery using evidence-based practices, creative problem-solving, and flexible approaches! 


As the CCM Coordinator, you will work closely with an experienced multidisciplinary team of empathetic direct care professionals committed to improving the health and wellness of their patients. Through thoughtful organization of daily operations, supportive administrative oversight, and strengths-based supervision, you will empower the team to deliver exceptional care and achieve outstanding patient outcomes! 

Requirements

Duties and Responsibilities:

As the Clinical Case Management Coordinator, you’ll play a key leadership role in supporting a passionate team that is making a real difference in the lives of individuals living with severe and persistent mental illness. Your day-to-day activities will include:

  • Leading, supervising, and mentoring a team of bachelor’s and master’s level clinicians, case managers, and peer support specialists.
  • Conducting performance appraisals, team meetings, individual supervision, and training of all new and existing staff on the team
  • Overseeing documentation audits, clinical quality, and program operations implementation of Evidence-Based Practices
  • Facilitating coordination of conditional discharges and participating in emergency consultations as      needed
  • Participating in agency-wide training, presentations, and quality improvement initiatives

Knowledge, Skills and Abilities:

  • Licensure: Active New Hampshire LICSW or LCMHC preferred but not required
  • Experience: Minimum 5 years in mental/behavioral health; supervisory experience preferred
  • Skills:
    • Strong leadership and communication abilities
    • Effective time management and organizational skills
    • Team-focused, yet able to work independently
    • Approachable support and strengths-based approach
    • Familiarity with CBT for Psychosis, recovery models, and crisis management
  • Other Requirements:
    • Comfortable using email, electronic records, and standard office software
