Essential Job Functions:
- Implementation of all programs/services as scheduled.
- Daily data recording on programs, activities, and outings in the community to report to QMRP.
- Meal preparation following menu. Consumers to assist in meal preparation with staff supervision and assistance as needed.
- Follow cleaning schedule and assuring facility is clean and safe.
- Attend in-services and other training as scheduled.
- Provide transportation for the individuals receiving services (work, activities, etc.).
- Authorized Direct Support Persons must supervise self-medication programs and administer medication as needed.
- Chart significant changes, behavior, illness, etc., in Universal Progress Notes.
- Document incident/accident reports and report them to the QIDP/RN.
- Other duties (file, shop, assist with pet care, etc.) as assigned, which are reasonably within the scope of the duties above.
Requirements
Basic Qualifications
- High School diploma or GED equivalent required.
- Valid Driver’s License
- Verified Background Check
- DSP Certified
- Passing of ADSP Program
Work Experience
Knowledge, Skills, and Requirements
- Must be able to satisfactorily complete a pre-employment background screen exam for this position, drug testing, as well as an annual body mechanics screening.
- Database knowledge is a plus.
- Must take and be trained in OIG Rule 50

- Ability to communicate in English via phone, in writing, and verbally in conversation with different levels of staff, resident families, and any outside contacts.
