Heartline Staffing, strategic talent partners with Purple Cow Recruiting, is seeking an experienced Director of Case Management to lead hospital-wide case management and discharge planning operations within an acute care setting.
The Director of Case Management has overall responsibility for managing and coordinating all aspects of hospital case management operations. This leader ensures compliance with organizational policies, regulatory requirements, and external agencies while integrating patient care and discharge planning processes across hospital departments and community partners to ensure continuity of care and optimal clinical resource utilization.
This role provides executive leadership for daily case management operations, including oversight of discharge planning, utilization review, interdisciplinary collaboration, and performance outcomes. The Director actively engages clinical teams and external stakeholders to achieve established quality, throughput, and length-of-stay goals while maintaining a patient-centered approach.
The position requires a strong executive presence, excellent communication and presentation skills, and a proven ability to motivate teams, deliver results, and drive performance in a fast-paced acute care environment.
Schedule:
Day shift, weekdays
May include on-call rotations or extended days as needed
Compensation:
Base salary ranges based on experience:
Minimum $109,762
Midpoint approximately $137,155 with 10 years of experience
Maximum up to $150,000 for candidates with 15 or more years of experience
A $10,000 sign-on bonus is available for non-HCA employees.

Requirements
Education:
- Bachelor’s degree required
- Master’s degree in Nursing, Health Administration, or Business Administration preferred
Licensure:
- Active Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) license required
Experience:
- Minimum of 3 years of recent acute care hospital case management experience
- Minimum of 2 years of leadership experience in hospital case management
- Hospital discharge planning experience required
- Recent acute care experience within the last year required
Leadership & Skills:
- Strong executive presence with excellent communication and presentation abilities
- Demonstrated success leading interdisciplinary teams
- Proven track record of achieving performance, quality, and utilization goals
