At Clinical Associates, we make being healthy easier. We connect our patients to a seamless system of integrated medical care, which includes in-house specialists in areas of practice, from cardiology to podiatry.
Our physicians, specialists and credentialed health care providers are assisted by 175 trained support staff. We do our best to take care of everything for our patients – medical records, scheduling appointments, coordinating with providers and specialists, and each of our offices has laboratory centers for tests and blood work on site.
We are seeking to add a Clinical Coordinator to our Quality/ Risk Management Team! Our ideal candidate is preferably a Medical Assistant that has worked in the clinical aspect of patient care with a focus in Internal Medicine or Primary Care. We are willing to consider a CNA/GNA with a strong primary care background. Excellent Microsoft Office skills are a must!
The Quality / Risk Management Clinical Coordinator will be responsible for the following duties including but not limited to:
- Maintaining patient panel listings by scheduling appointments, follow ups, and closing gaps in care.
- Formatting the hospital based Encounter Notification System reports, scheduling appointments for Transitional Care Management, Annual Wellness Visits, and other requested campaigns.
- Supporting the Manager of Quality and Risk Management with departmental reports on patient compliance as requested by insurance payors, company administrators, and healthcare providers.
- Covering team members as needed.
Shift: Full Time M -F 8:30am - 4:30pm
Requirements:
- High School Diploma or G.E.D.
- Medical Assistant experience preferred - Internal Medicine / Primary Care
- 2 years of Medical Office experience preferred.
- Excellent patient care and customer service skills.
- Proficient EMR / computer skills with a willingness and ability to learn new applications.
